A Ag-Cu-Mo-Bi-W-Sb mine located in sec. 19, T24N, R17E (MDM), 2.9 km (1.8 miles) WNW of Adams Peak (coordinates of record), SE of Galeppick Creek, on the W slope of the Diamond Mountains, E of Frenchman Lake and the Little Last Chance Valley, SE of Last Chance Spring, on National Forest land.

NOTES:
- This locality is not listed in the USGS MRDS database.
- Part of the Lodi 4 mine is located on a parcel of private land within the national forest.

Mineralization is quartz dikes in granite related to either hydrothermal phase of magmatic sequence or pneumatolytic origin.
