Comparison of Right Ventricle Systolic Function between Long‐Term Anabolic–Androgenic Steroid User and Nonuser Bodybuilder Athletes: A Study of Two‐Dimensional Speckle Tracking Echocardiography

Abstract:Despite normal standard systolic echo parameters, peak systolic RV free wall strain and strain rate were reduced in AAS user bodybuilders in comparison with nonusers. Strain and strain rate by 2DSTE may be useful for early determination of subclinical RV dysfunction in AAS user bodybuilders.

“…Subsequently, we reported reduced left ventricular ejection fractions and ventricular diastolic impairments in middle-aged long-term users of supraphysiologic-dose AAS(Baggish et al, 2010), effects that we and others confirmed in subsequent larger-scale studies(Angell et al, 2012;Luijkx et al, 2013;Baggish et al, 2017;Rasmussen et al, 2018a). Other groups also have reported finding right ventricular abnormalities(Angell et al, 2014;Alizade et al, 2016;Rasmussen et al, 2018a), apoptosisrelated fibrotic changes in the heart(Cecchi et al, 2017), increased systolic blood pressure and aortic stiffness(Rasmussen et al, 2018b), and a pro-coagulant state(Chang et al, 2018) in long-term supraphysiologic-dose AAS users. In an echocardiography study in male rabbits, 6 months of supraphysiologic nandrolone administration (SC, 4 or 10 mg/kg, twice/ week) was associated with impaired global myocardial performance in a dose-related manner(Vasilaki et al, 2016).…”

“…In our study, transthoracic echocardiograms were performed in 18.5% men over 35 and in 8.8% of younger patients. Given the increased frequency of left ventricular hypertrophy (LVH), hypertension, and atherosclerosis seen with AAS use(Alizade et al, 2016;Baggish et al, 2017;Thiblin et al, 2015), our data suggest that TTEs are likely underutilized in this population. The lack of additional indicated testing for this population supported a prior study, concluding that physicians do not have adequate knowledge of AAS(Hill & Waring, 2019).…”

“…While long-term prospective harm data are lacking, strong evidence exists for the development of accelerated atherosclerosis, left ventricular hypertrophy, arrhythmias, hypertension, and HDL suppression ( Achar et al, 2010 ; Alizade et al, 2016 ; Baggish et al, 2017 ; Santora et al, 2006 ; Thiblin et al, 2015 ). Polycythemia, infertility, tendon rupture of the upper extremity, and behavioral disorders are commonly associated with AAS use ( Coviello et al, 2008 ; Hauger et al, 2019 ; Kanayama, DeLuca, et al, 2015 ; Rahnema et al, 2014 ; Tatem et al, 2019 ; Thiblin et al, 1999 ).…”

“…Baumann et al reported myocardial scaring and cardiomegaly, diagnosed by MRI in a male bodybuilder with 20 years of AS use[35]. Alizade et al found an increase in right ventricular heart strain with the use of a two-dimensional speckle tracking echocardiography[36]. On the other hand, D'Andrea et al found left atrial dysfunction with the use of speckle echocardiography in AS users[37].…”
