    To help eliminate perinatal HIV transmission, the US Department of Health and Human Services recommends against breastfeeding for women living with HIV, regardless of viral load or combined antiretroviral therapy status. However, cART radically improves HIV prognosis and virtually eliminates perinatal transmission, and breastfeeding's health benefits are well-established. In this setting, pregnancy is increasing among American women with HIV, and a harm reduction approach to those who breastfeed despite extensive counseling is suggested. We assess the evidence and ethical justification for (...) current policy, with attention to pertinent racial and health disparities. We first review perinatal transmission and breastfeeding data relevant to US infants. We compare hypothetical risk of HIV transmission from breastmilk to increased mortality from sudden infant death syndrome, necrotizing enterocolitis and sepsis from avoiding breastfeeding, finding that benefits may outweigh risks if mothers maintain undetectable viral load on cART. We then review maternal health considerations. We conclude that avoidance of breastfeeding by women living with HIV may not maximize health outcomes and discuss our recommendation for revising national guidelines in light of autonomy, harm reduction and health inequities.     Teaching allows human culture to exist and to develop. Despite its significance, it has not been studied in depth by the cognitive neurosciences. Here we propose two hypotheses to boost the claim that teaching is a human instinct, and to expand our understanding of how teaching occurs as a dynamic bi-directional relation within the teacher-learner dyad. First, we explore how children naturally use ostensive communication when teaching; allowing them to be set in the emitter side of natural pedagogy. Then, we (...) hypothesize that the capacity to teach may precede to even have a mature metacognition and, we argue that a teacher will benefit from the interaction with her student, improving her understanding on both contents of knowledge: her own and her student’s. Thus, we propose that teaching may be the driving force of metacognitive development and may be occurring as an instinct from very early ages. (shrink)

    Myotonic dystrophy type 1 (DM1) is considered a progeroid disease (i.e., causing premature aging). This hypervariable disease affects multiple systems, such as the musculoskeletal, central nervous, gastrointestinal, and others. Despite advances in understanding the underlying pathogenic mechanism of DM1, numerous gaps persist in our understanding, hindering elucidation of the heterogeneity and severity of its symptoms. Accumulating evidence indicates that the toxic intracellular RNA accumulation associated with DM1 triggers cellular senescence. These cells are in a state of irreversible cell cycle arrest (...) and secrete a cocktail of cytokines, referred to as a senescence‐associated secretory phenotype (SASP), that can have harmful effects on neighboring cells and more broadly. We hypothesize that cellular senescence contributes to the pathophysiology of DM1, and clearance of senescent cells is a promising therapeutic approach for DM1. We will discuss the therapeutic potential of different senotherapeutic drugs, especially senolytics that eliminate senescent cells, and senomorphics that reduce SASP expression. (shrink)

    While freedom of movement has been one of the most highly respected human right across the EU, there are various aspects which come into play which still need to be resolved for this to be achieved in practice. One of these key issues is cross border health care. Indeed, there is an increasing awareness of standardisation of health service provision and cross border collaboration in the EU. However, certain groups particularly children may be at risk of suboptimal treatment as a (...) result. We present the case of a child patient which highlights the complexity of this matter spanning family law, health law, social security law as well as ethical issues.     RESUMO Este texto apresenta parte de pesquisa recém-concluída, que delineia dimensões e balizas para o que vem sendo denominado perspectiva discursiva de alfabetização. Princípios da teoria da enunciação e de estudos que analisam as relações entre sujeito e linguagem e o papel das interações discursivas nos processos de ensino e aprendizagem organizam a base teórico-metodológica do estudo, em que se analisam relatos escritos por cinco professoras alfabetizadoras durante 18 meses. Duas dimensões teórico-metodológicas de análise são trabalhadas: discursiva e indiciária. Mergulha-se (...) nos modos como a prática pedagógica é movimentada em sala de aula, em busca de indícios para descrever a situação discursiva em que foram colhidos, analisá-los e interpretá-los. A pesquisa tem como horizonte compreender como podem ser concebidas metodologias de alfabetização em que o trabalho com a vida das crianças na cultura escrita se sobreponha ao trabalho tradicionalmente realizado com foco no sistema de escrita. ABSTRACT This paper presents data from a recently finished research that outlines dimensions and guidelines for what has been named discursive perspective on literacy. Principles of the theory of enunciation and studies that analyze the relationships between subject and language as well as the role of discursive interactions in the teaching and learning processes, organize the theoretical and methodological basis of the study. We analyzed reports written by five literacy teachers for 18 months. Two theoretical-methodological dimensions of analysis were employed, the discursive and the evidential. We focus on the ways the pedagogical practice is developed in the classroom, seeking for indexes to describe the discursive situation from which they were collected, and to analyze and interpret them. The aim of the research was to understand how to construct literacy methodologies in which working with children's lives in written culture overcomes the traditional work focused on the writing system.     Biomedical ontologies are emerging as critical tools in genomic and proteomic research where complex data in disparate resources need to be integrated. A number of ontologies exist that describe the properties that can be attributed to proteins; for example, protein functions are described by Gene Ontology, while human diseases are described by Disease Ontology. There is, however, a gap in the current set of ontologies—one that describes the protein entities themselves and their relationships. We have designed a PRotein Ontology (PRO) (...) to facilitate protein annotation and to guide new experiments. The components of PRO extend from the classification of proteins on the basis of evolutionary relationships to the representation of the multiple protein forms of a gene (products generated by genetic variation, alternative splicing, proteolytic cleavage, and other post-translational modification). PRO will allow the specification of relationships between PRO, GO and other OBO Foundry ontologies. Here we describe the initial development of PRO, illustrated using human proteins from the TGF-beta signaling pathway. (shrink)

    Filial Obligations: A Comparative Study.Cecilia Wee -2014 -Dao: A Journal of Comparative Philosophy 13 (1):83-97.
    The nature of the special obligation that a child has towards her parent(s) is widely discussed in Confucianism. It has also received considerable discussion by analytic commentators. This essay compares and contrasts the accounts of filial obligation found in the two philosophical traditions. The analytic writers mentioned above have explored filial obligations by relating them to other special obligations, such as obligations of debt, friendship, or gratitude. I examine these accounts and try to uncover the implicit assumptions therein about the (...) scope and nature of such filial obligations. I then similarly examine Confucian filial obligations (CFOs) by relating them to these other special obligations of debt, gratitude, and so on. My findings are used to highlight crucial differences in the scope and conception of filial obligations in these two traditions. (shrink)

    Previous studies on parenting adult children with ASD were scarce, and their intervention protocols mainly were derived from established work with children. Development of an applicable adult-oriented protocol and demonstration of its effectiveness is warranted. The present study outlined the development and evaluation of Core Autism Parenting Skills, which targets to enhance parenting self-efficacy intervention for adult children with ASD by addressing two intervention goals in parallel: acquisition of parenting skills and cultivating positive attributes. In CAPS, PSE is operationalised into (...) four parent roles: to observe, reinforce, empathise, and accompany, each with requisite attributes, skills, and prescribed training. Twenty-seven parents with adult children with ASD were recruited. They completed measures assessing their PSE, competence in the four parent roles, and emotional well-being at pre-training, post-training and 2-month follow-up. The intervention was well-received by the participants and reported significant improvements in PSE, parent role competence at post-training and 2-month follow-up. The applicability of PSE and parent role competence in constructing effective parenting intervention for adult children with ASD was supported. (shrink)

Protein entities represented in PRO corresponding to single amino acid chains are categorized by level of specificity into family, gene, sequence and modification metaclasses, and there is a separate metaclass for protein complexes. All metaclasses also have organism-specific derivatives. PRO complements established sequence databases such as UniProtKB, and interoperates with other biomedical and biological ontologies such as the Gene Ontology (GO). PRO relates to (...) UniProtKB in that PRO’s organism-specific classes of proteins encoded by a specific gene correspond to entities documented in UniProtKB entries. PRO relates to the GO in that PRO’s representations of organism-specific protein complexes are subclasses of the organism-agnostic protein complex terms in the GO Cellular Component Ontology. The past few years have seen growth and changes to the PRO, as well as new points of access to the data and new applications of PRO in immunology and proteomics.     We examine the comportment of the global trajectory of a piecewisely conceived single species population growth model. Formulation relies on what we develop as the principle of limiting factors for population growth, adapted from the law of the minimum of Liebig and the law of the tolerance of Shelford. The ensuing paradigm sets natality and mortality rates to express through extreme values of population growth determining factor. Dynamics through time occur over different growth phases. Transition points are interpreted as thresholds (...) of viability, starvation, and intraspecific competition. In this delivery, we focus on the qualitative study of the global trajectory expressed on continuous time and on exploring the feasibility of analytical results against data on populations growing under experimental or natural conditions. All study cases sustained fittings of high reproducibility both at empirical and interpretative slants. Possible phase configurations include regimes with multiple stable equilibria, sigmoidal growth, extinction, or stationarity. Here, we also outline that the associating discrete-time piecewise model composes the logistic map applied over a particular region of the phase configuration. Preliminary exploratory analysis suggests that the logistic map’s chaos onset could surpass once the orbit enters a contiguous phase region. (shrink)

Pedagogical practice : This article’s main purpose is to present a few possibilities of work with activities that involve pieces of children’s literature and music in the perspective of Children’s Education and first years of Primary School. As theoretical support, the paper presents several studies from authors such as Brito (2003), Ponso (2008), Souza (2006), Torres and Gallicchio (2004), among others who suggest developing activities of sound exploration through audition/appreciation and improvise/ sound editing (...) from some texts and books of children’s literature. A conclusion has been reached that the proposal of activities transforming words in music enables readers to experiment different sounds to compose the soundtrack of stories. Besides, when selecting a diversified repertoire with several texts, possibilities are created to the students to tell, recount and sing varied stories, allowing, thus, the entrance of different social voices. : Literatura Infantil. Prática pedagógica. : O objetivo maior deste artigo é o de apresentar algumas possibilidades de trabalho com atividades que envolvam obras de literatura infantil e músicas, na perspectiva da Educação Infantil e Séries Iniciais do Ensino Fundamental. Como suporte teórico, o trabalho apresenta diversos estudos de autores, tais como Brito (2003), Ponso (2008), Souza (2006),Torres e Gallicchio (2004) entre outros, que sugerem desenvolver atividades de exploração sonora através da audição/ apreciação e improvisação/sonorização a partir de alguns textos e de livros de literatura infantil. Constatou-se que a proposta de atividades, transformando palavras em músicas, possibilita interpelar os leitores a experimentarem diferentes sons para compor a trilha sonora de histórias. Além disso, ao selecionar um repertório diversificado com vários textos, criam-se possibilidades aos alunos para contarem, recontarem e cantarem variadas histórias, permitindo, dessa maneira, a entrada de diferentes vozes sociais.     Alzheimer’s disease (AD) may be situated within a cultural landscape produced, in part, by demographics and the marketing strategies of an aggressive biopharmaceutical industry. The simultaneously corporeal and visual domain of advertisements for anti-AD drugs generates dynamic images of gender and embodiment, and it also lends itself to feminist interventions engaging with the images and ideas circulating around aging, medicine and the body. In this article, we investigate advertisements targeting medical practitioners treating patients with AD. Working within a methodological framework (...) we identify as ‘feminist visual studies of technoscience’, we want to propel the discussion in the direction of a broader corpus of medical media. Through this limited exercise, we hope to make a scholarly contribution to the feminist community by critiquing some of the images emerging within popular/scientific media with regard to Alzheimer’s, a disease collectively imagined within an aging Western population.     The procedures for structuring clinical ethics case reflections in a childhood cancer care setting are presented, including an eight-step model. Four notable characteristics of the procedures are: members of the inter-professional health care team, not external experts, taking a leading role in the reflections; patients or relatives not being directly involved; the model explicitly addressing values and moral principles instead of focussing exclusively on the interests of involved parties; using a case-based (inductive) rather than principle-based (deductive) method. By discusing the (...) advantages and disadvantages of the proposed procedures, our paper aims to contribute to the literature on models and procedures for ethical analysis that can be used in clinical settings. It is suggested that our proposed procedures have some advantageous features when it comes to promoting health care staff’s learning to structure their thinking about ethical issues. (shrink)

    Rawls’ theory of justice has been exposed to several criticisms, most of them having been supported on some counter-intuitive results. The intent of this article is to show that these criticisms fail to affect the Rawlsian model, since, as a theoretical construction of the concept of justice, it is doomed to account for potential, rather than actual, empirical contents.

    This paper aims at emphasizing the relevance of scrutinizing the link between Heidegger’s theory of Being and his Nazi commitment. Significant investigations of such a link, beginning with that of Victor Farias, have challenged the „official” view that Heidegger’s theory of Being is politically neutral. That this theory could not have been deprived of political intentions is proven by Heidegger’s own view on philosophy as a search that stems from life and ends in life, namely as a Dasein’s way of (...) life. Since fundamental ontology (theory) and existential analytic (practice) cannot be separated, exploring the relationship between them is even more necessary. (shrink)

    Background: How well ethical concerns are handled in healthcare is influenced by the ethical climate of the workplace, which in this study is described as workplace factors that contribute to healthcare professionals’ ability to identify and deal with ethical issues in order to provide the patient with ethically good care. Objectives: The overall aim of the study was to describe perceptions of the paediatric hospital ethical climate among healthcare professionals who treat/care for children with cancer. Research design: Data were collected (...) using the Hospital Ethical Climate Survey developed by Olsson as a separate section in a questionnaire. Descriptive statistics were used to analyse perceptions of the ethical climate. Participants and research context: Physicians, nurses and nurse-aides (n = 89) from three paediatric units participated in this study: haematology/oncology, chronic diseases and neurology. Ethical considerations: The study was approved by the regional ethical review board. Findings: Different perceptions of the ethical climate were rated as positive or negative/neutral. Nurses’ ratings were less positive than physicians on all items. One-third of the participants perceived that they were able to practice ethically good care as they believed it should be practised. Discussion: Differences in professional roles, involving more or less power and influence, might explain why physicians and nurses rated items differently. A positive perception of the possibility to practice ethically good care seems to be related to inter-professional trust and listening to guardians/parents. A negative/neutral perception of the possibility to practice ethically good care appears to be influenced by experiences of ethical conflicts as well as a lack of ethical support, for example, time for reflection and discussion. Conclusion: The two-thirds of participants who had a negative/neutral perception of the possibility to practice ethically good care are at risk of developing moral stress. Clinical ethics support needs to be implemented in care where important values are at stake. (shrink)

    Background: Childhood cancer care involves many ethical concerns. Deciding on treatment levels and providing care that infringes on the child’s growing autonomy are known ethical concerns that involve the whole professional team around the child’s care. Objectives: The purpose of this study was to explore healthcare professionals’ experiences of participating in ethics case reflection sessions in childhood cancer care. Research design: Data collection by observations, individual interviews, and individual encounters. Data analysis were conducted following grounded theory methodology. Participants and research (...) context: Healthcare professionals working at a publicly funded children’s hospital in Sweden participated in ethics case reflection sessions in which ethical issues concerning clinical cases were reflected on. Ethical considerations: The children’s and their parents’ integrity was preserved through measures taken to protect patient identity during ethics case reflection sessions. The study was approved by a regional ethical review board. Findings: Consolidating care by clarifying perspectives emerged. Consolidating care entails striving for common care goals and creating a shared view of care and the ethical concern in the specific case. The inter-professional perspectives on the ethical aspects of care are clarified by the participants’ articulated views on the case. Different approaches for deliberating ethics are used during the sessions including raising values and making sense, leading to unifying interactions. Discussion: The findings indicate that ethical concerns could be eased by implementing ethics case reflection sessions. Conflicting perspectives can be turned into unifying interactions in the healthcare professional team with the common aim to achieve good pediatric care. Conclusion: Ethics case reflection sessions is valuable as it permits the discussion of values in healthcare-related issues in childhood cancer care. Clarifying perspectives, on the ethical concerns, enables healthcare professionals to reflect on the most reasonable and ethically defensible care for the child. A consolidated care approach would be valuable for both the child and the healthcare professionals because of the common care goals. (shrink)
