    State Medical Boards (SMBs) can take severe disciplinary actions (e.g., license revocation or suspension) against physicians who commit egregious wrongdoing in order to protect the public. However, there is noteworthy variability in the extent to which SMBs impose severe disciplinary action. In this manuscript, we present and synthesize a subset of 11 recommendations based on findings from our team’s larger consensus-building project that identified a list of 56 policies and legal provisions SMBs can use to better protect patients from egregious (...) wrongdoing by physicians.     Artificial intelligence, as a nonhuman entity, is increasingly used to inform, direct, or supplant nursing care and clinical decision‐making. The boundaries between human‐ and nonhuman‐driven nursing care are blurred with the advent of sensors, wearables, camera devices, and humanoid robots at such an accelerated pace that the critical evaluation of its influence on patient safety has not been fully assessed. Since the pivotal release of To Err is Human, patient safety is being challenged by the dynamic healthcare environment like never (...) before, with nursing at a critical juncture to steer the course of artificial intelligence integration in clinical decision‐making. This paper presents an overview of artificial intelligence and its application in healthcare and highlights the implications which affect nursing as a profession, including perspectives on nursing education and training recommendations.     Plato's thought evolves from the epistemology of the Meno, Phaedo, and Republic to the Combined Doctrine of the Theaetetus. The Combined Doctrine maintains that both Forms and certain objects rooted in perception are objects of knowledge. Dialectic results in apprehension of the Good, and consequently of being, which brings about a permanent change in a person's state of mind enabling one to know what one previously believed. The Combined Doctrine resolves the paradoxes of the refutations of the Theaetetus.     Tigard (2019) suggests that the medical community would benefit from continuing to promote notions of individual responsibility and blame in healthcare settings. In particular, he contends that blame will promote systematic improvement, both on the individual and institutional levels, by increasing the likelihood that the blameworthy party will ‘own up’ to his or her mistake and apologise. While we agree that communicating regret and offering a genuine apology are critical steps to take when addressing patient harm, the idea that medical (...) professionals should continue to ‘take the blame’ for medical errors flies in the face of existing science and threatens to do more harm than good. We contrast Dr Tigard’s approach with the current literature on blame to promote an alternative strategy that may help to create lasting change in the face of unfortunate error. (shrink)

    Trafficking and persistence of fetal microchimeric cells (fMCs) and circulating extracellular vesicles (EVs) have been observed in animals and humans, but their consequences in the maternal body and their mechanistic contributions to maternal physiology and pathophysiology are not yet fully defined. Fetal cells and EVs may help remodel maternal organs after pregnancy‐associated changes, but the cell types and EV cargos reaching the mother in preterm pregnancies after exposure to various risk factors can be distinct from term pregnancies. As preterm delivery‐associated (...) maternal complications are rising, revisiting this topic and formulating scientific questions for future research to reduce the risk of maternal morbidities are timely. Epidemiological studies report maternal cardiovascular risk as one of the major complications after preterm delivery. This paper suggests a potential link between fMCs and circulating EVs and adverse maternal cardiovascular outcomes post‐pregnancies, the underlying mechanisms, consequences, and methods for and how this link might be assessed.     Formalized Responsible Conduct of Research (RCR) programs have become a compliance requirement. Yet evidence consistently demonstrates that compliance-based ethics training focused on teaching regulations and “rules” fails to create ethical cultures. Research and practice in behavioral ethics have demonstrated that there is value in moving away from rule-based, normative, ethics education toward approaches rooted in descriptive explainations about how and why individuals make unethical decisions, and focused on environmental and cultural influences. We examine the circumstances—and subsequent assumptions—that lead to compliance-based (...) RCR training, unpack those assumptions, and identify their outcomes. The question at the heart of compliance-based training is “What must we teach people so they will make ethical decisions?” The command-and-control nature of this question fails to encompass needed elements for broader research integrity, namely, developing ethical habits and building skills for ethical decision-making. Asking instead “How do we develop people to articulate and act in alignment with ethical values?” helps to shift toward more effective and sustainable ethics education. Educating about ethical decision-making develops people who are more practically wise and who can influence the cultures of the environments in which they work. We conclude by offering examples of ethics education and development that are asking the right question and include information about our own leadership development program for elite laboratory scientists that is seeking to support ethical and healthy working cultures.     In the fifth–sixth century CE the rulers of the Kadamba dynasty claimed the town of Halsi in modern Karnataka as the northern capital of their expanding polity. Their investments in this locale are recorded in a corpus of copper-plate inscriptions spanning four generation of kings. The plates record the growth of a thriving Jain community at Palāśikā and are revelatory of their relationships with the Kadamba rulers and their agents. This study of the donative and political processes converging in Palāśikā (...) shows that the use of Sanskrit inscriptions as media for royal representation and public self-fashioning was highly developed in the Kadamba polity, where idioms and trends developed independent of the Gupta royal model. Moreover, the evidence from Halsi is indicative of the centrality of Jain religious communities, ideologies, and institutions in the administration of the Kadamba polity and the expression of a lineage identity.     One hundred and three participants solved conflict and non-conflict versions of four reasoning tasks using a two-response procedure: a base rate task, a causal reasoning task, a denominator neglect task, and a categorical syllogisms task. Participants were asked to give their first, intuitive answer, to make a Feeling of Rightness judgment, and then were given as much time as needed to rethink their answer. They also completed a standardized measure of IQ and the actively open-minded thinking questionnaire. The FORs of (...) both high- and low-capacity reasoners were responsive to conflict, such that FORs were lower for conflict relative to non-conflict problems. Consistent with the quantity hypothesis, high-capacity reasoners made a greater distinction between conflict and non-conflict items on measures of Type 2 thinking, namely, rethinking time and probability of changing answers. In contrast to the quality hypothesis, however, this rethinking time did not advantage the ability of the high-capacity group to produce normative answers, except for the base rate task. Indeed, we observed that the correlation between capacity and the probability of normative answers emerged at the initial response, rather than after rethinking. (shrink)

    Under some circumstances, individuals are willing to engage in unethical behaviors that benefit another entity. In this research we advance the unethical pro-organizational behavior construct by showing that individuals also have the potential to behave unethically to benefit their supervisors. Previous research has not examined if employees engage in unethical acts to benefit an entity that is separate from oneself and if they will conduct these acts to benefit a supervisor. Our research helps to address these gaps. We also demonstrate (...) that unethical behavior to benefit a supervisor, what we term unethical pro-supervisor behavior, is more likely to occur if individuals are more (versus less) identified with their organization or supervisor. That is, feeling a sense of oneness with one’s organization or supervisor can result in employees engaging in unethical behavior to help their supervisor. Further, this positive relationship is weakened if the employee possesses higher levels of moral identity. We test our hypotheses with a two-part laboratory study, a field study, and a time-lagged field study. Theoretical and practical implications of this work are discussed. (shrink)

(Hellenistic Culture and Society, 22)What was asylia, and what did the numerous grants of it signify? Kent Rigsby has tackled this 300-year-old question by compiling the first-ever collection of asylia decrees and coin legends, (...) thereby performing an immense service for those interested in Hellenistic diplomacy and religion, and putting the study of asylia on an entirely new footing.Asylia is “the declared inviolability of places” (19), not persons, granted by “Panhellenic recognition” (21), and embodying “first and foremost a religious gesture, increasing the honor of the god” (14). The body of material about asylia begins in the 260s B.C. and ends in A.D. 22–23, when the Romans reviewed claims to asylia (Tac. 3.60–63, 4.14); references to asylia thereafter are not to new grants, but to grants made before A.D. 23 (3, 29, 35). This declared inviolability of places includes both temples and cities, for “the inscriptions... by their language, geography, and chronology imply that asylia of temple and [End Page 460] city was the same” (20). As preface to the corpus there is a brief introduction (1–29), which surveys existing explanations of asylia, and an even briefer chapter called “The Documents” (30–40), which discusses terminology and the restrictions on the corpus itself. Finally, there is another brief chapter that looks at the mythical or quasi-historical examples of asylia at Elis, Delphi, Plataea, and Delos (41–53).In the corpus of 230 epigraphic entries that follows, the phenomenon of asylia in each area or city is discussed in its own chapter or section, parallel or supporting material thoughtfully examined, and the coin evidence scrutinized. This is not a book for beginners, as its policy of selective translation (40) and its lack of maps demonstrate. (All who know where Rhaucus and Baetocaece are situated, raise your hands!) The erudition is enormous, the scholarship meticulous. Even the fearsome editors of the Bulletin Epigraphique quailed at reporting all of Rigsby’s corrections and plausible conjectures and meekly recorded only five footnote-sized adjustments of their own. (Two of these, it should be noted, were based on information published too late to have come to Rigsby’s attention anyway; see REG 110 [1997] 484–85.) His grasp of detail and relentless good judgment must and will disarm all but the most aggressively knowledgeable critic with strong opinions on small matters. Louis Robert I am not.The question that seems to me worth asking, however, is: What can be made of this material? Rigsby, although he decided that “a corpus of the evidence would be of more use to scholars than another essay of historical interpretation” (xiii), does not neglect the necessary act of evaluating the significance of asylia. I do, however, question some of his conclusions, while emphasizing strongly that only his meticulous collection and study of the inscriptions make possible my qualified skepticism and the further questions I would like to see asked of this material.The introduction (1–29) critiques previous explanations of asylia from Spanheim (1706) to Pritchett (1991) and offers Rigsby’s own (13–19, 22–29). As he says, declarations of inviolability can hardly have been attempts to enact the proper (and traditional) immunity of “sacred or neutral places” when popular religious scruple was no longer upholding it: there seems little likelihood that what was not instinctively felt would be voluntarily observed in obedience to a mere declaration (15). Nor were grants of asylia promises of collective defense (against, e.g., pirates), for no such collective defense was ever mounted, those granting asylia were often in no position to offer useful assistance, and no oath—“the sine qua non of a serious military intent” (22–23)—secured compliance (16).So far, so good. But what function, then, did grants of asylia serve?     Although research integrity practices in institutional settings is not a new area of study, because of its foundational importance in university settings it remains a topic worthy of study. In addition, rarely are all members of the university community included as participants in studies focused upon research integrity and ethics. Thus, to add to the existent literature, the authors investigated research integrity practices in a medium-sized Midwestern polytechnic university setting, including 467 participants from across all divisions of the university community. (...) This mixed data survey study was comprised of six sections; presented is information for two sections—sample demographics and research integrity. The demographics appear reflective of those of the larger survey, as well as the university setting of study. In the research integrity section there were two parts—one qualitative and one quantitative.     The earlier volume in this series dealt with two religions of Indian origin, namely, Buddhism and Jainism. The Indian religious scene, however, is characterized by not only religions which originated in India but also by religions which entered India from outside India and made their home here. Thus religious life in India has been enlivened throughout its history by the presence of religions of foreign origin on its soil almost from the very time they came into existence. This volume covers (...) three such religions—Zoraoastrianism, Judaism, and Islam. In the case of Zoraostianism, even its very beginnings are intertwined with India, as Zoroastrianism reformed a preexisting religion which had strong links to the Vedic heritage of India. This relationship took on a new dimension when a Zoroastrian community, fearing persecution in Persia after its Arab conquest, sought shelter in western India and ultimately went on to produce India’s pioneering nationalist in the figure of Dadabhai Naoroji ( 1825-1917), also known as the Grand Old Man of India. Jews found refuge in south India after the destruction of the Temple by the Romans in 70 C.E. and have remained a part of the Indian religious scene since then, some even returning to Israel after it was founded in 1948. Islam arrived in Kerala as soon as it was founded and one of the earliest mosques in the history of Islam is found in India. Islam differs from the previously mentioned religions inasmuch as it went on to gain political hegemony over parts of the country for considerable periods of time, which meant that its impact on the religious life of the subcontinent has been greater compared to the other religions. It has also meant that Islam has existed in a religiously plural environment in India for a longer period than elsewhere in the world so that not only has Islam left a mark on India, India has also left its mark on it. Indeed all the three religions covered in this volume share this dual feature, that they have profoundly influenced Indian religious life and have also in turn been profoundly influenced by their presence in India. Although the field of medical ethics is by nature eclectic, moral philosophy has played a dominant role in defining both the content of what is taught and the methodology for reasoning about ethical dilemmas. Most educators largely rely on the case‐based method for teaching ethics, grounding the ethical reasoning in an amalgam of theories drawn from (...) moral philosophy, including consequentialism, deontology, and principlism.In this article we hope to make a case for augmenting the focus of education in medical ethics. We propose complementing the traditional approach to medical ethics with a more embedded approach, one that has been described by others as “microethics,” the ethics of everyday clinical practice.     We investigated how age of faces and emotion expressed in faces affect young (n=30) and older (n=20) adults’ visual inspection while viewing faces and judging their expressions. Overall, expression identification was better for young than older faces, suggesting that interpreting expressions in young faces is easier than in older faces, even for older participants. Moreover, there were age-group differences in misattributions of expressions, in that young participants were more likely to label disgusted faces as angry, whereas older adults were more (...) likely to label angry faces as disgusted. In addition to effects of emotion expressed in faces, age of faces affected visual inspection of faces: Both young and older participants spent more time looking at own-age than other-age faces, with longer looking at own-age faces predicting better own-age expression identification. Thus, cues used in expression identification may shift as a function of emotion and age of faces, in interaction with age of participants. (shrink)

    Background: When adolescent boys experience close, secure relationships with their parents and peers, the implications are potentially far reaching, including lower levels of mental health problems in adolescence and young adulthood. Here we use rare prospective intergenerational data to extend our understanding of the impact of adolescent attachments on subsequent postpartum mental health problems in early fatherhood.Methods: At age 17–18 years, we used an abbreviated Inventory of Parent and Peer Attachment to assess trust, communication, and alienation reported by 270 male (...) participants in their relationships with mothers, fathers, and peers. More than a decade later, we assessed the adult males, now fathers, at 12 months postpartum for symptoms of depression, anxiety, and stress. Logistic regression was used to examine the extent to which attachment dimensions predicted paternal postpartum mental health, adjusting for potential confounding, and with assessment for interactions between parent and peer attachments.Results: Trust in mothers and peers, and good communication with fathers during adolescence, were associated with 5 to 7 percentage point reductions in postpartum mental health symptoms in early fatherhood. Weak evidence of parent-peer interactions suggested secure attachments with either parent or peer may compensate for an insecure attachment with the other.Conclusions: Our results suggest that fostering trust and communication in relationships that adolescent boys have with parents and peers may have substantial effects on rates of paternal postpartum mental health problems. The protective benefits may be preventative in intergenerational cycles of risk for mental health problems. (shrink)

    One of the challenges of modern healthcare ethics practice is the navigation of boundaries. Practicing healthcare ethicists in the performance of their role must navigate meanings, choices, decisions and actions embedded in complex cultural and social relationships amongst diverse individuals. In light of the evolving state of modern healthcare ethics practice and the recent move toward professionalization via certification, understanding boundary navigation in healthcare ethics practice is critical. Because healthcare ethics is endowed with many boundaries which often delineate concerns about (...) professional expertise and authority, epistemological reflection on the relationship between theory and practice points toward the social context as relevant to the conceptualization of boundaries. The skills of social scientists may prove helpful to provide data and insights into the conceptualization and navigation of clinical ethics qua profession. Empirical ethics research, which combines empirical description (usually social scientific) with normative-ethical analysis and reflection, is a way forward as we engage and reflect upon issues which have implications for practice standards and professionalization of the role. This requires cooperative engagement of the descriptive and normative disciplines to explore our understandings of boundaries in healthcare ethics practice. This will contribute to the ongoing reflection not only as we envision the professional role but to ensure that it is enacted in practice. (shrink)
