Abstract

A modular component for a knee joint prosthesis is disclosed. The modular component includes a tray, a stem and an adapter assembly. The tray includes a support surface and downwardly extending extension having a generally circular shape. The stem includes a main body portion and an upwardly extending extension. The adapter assembly connects the tray and the stem. The adapter assembly includes a first generally cylindrical cavity receiving the downwardly extending extension of the tray and a second generally cylindrical cavity receiving the upwardly extension of the stem.

Description

CROSS-REFERENCE TO RELATED APPLICATIONS
• This is a divisional of U.S. patent application Ser. No. 10/289,585 filed on Nov. 7, 2002, which is a continuation-in-part application of U.S. patent application Ser. No. 09/792,172, filed Feb. 23, 2001. Each of these disclosures are incorporated herein by reference.
FIELD
• The present teachings relate generally to a joint prosthesis and more particularly to a knee joint prosthesis having a modular tibial component with an offset tibial stem.
BACKGROUND
• A knee joint prosthesis typically comprises a femoral component and a tibial component. The femoral component and tibial component are designed to be surgically attached to the distal end of the femur and the proximal end of the tibia respectively. The femoral component is further designed to cooperate with the tibial component in simulating the articulating motion of an anatomical knee joint. Knee joint prostheses, in combination with ligaments and muscles, attempt to duplicate natural knee motion as well as absorb and control forces generated during the range of flexion.
• While known knee joint prostheses have proven to be effective in replacing the anatomical knee joint, they nevertheless have several disadvantages. For example, knee joint prostheses sometimes lack interchangeability between a femoral component designed specifically for a right knee or a left knee and a particular component. In this regard, in a normally shaped tibia, the central canal is typically offset from the center of the tibial articulating surfaces or the center of the tibial plateau. The stems of most prior tibial implants have been positioned centrally to the implant base or tibial tray. Although a central location of the stem allows for particular implant to be used for either the right or left knee, such a stem position is associated with drawbacks. The primary drawback is that the centrally located stem was substantially offset from the center of the tibial canal itself when the base of the implant was aligned with the resected tibial surface.
• To a more limited extent, it is also known to provide a knee joint prosthesis with an offset tibia stem. While knee joint prosthesis with offset tibial stems provide certain identified advantages, they nevertheless can be the subject of certain improvement.
SUMMARY
• The present teachings relate to a tibial component for a knee joint prosthesis having an offset stem.
• A tibial component of a knee joint prosthesis having a common tibial tray and a plurality of tibial stems with various offsets for selectively engaging the tibial tray is disclosed.
• A tibial component of a knee joint prosthesis having a offset modular stem that securely engages a tibial tray is disclosed.
• A modular component of a knee joint prosthesis that permits different degrees of stem offset with minimal inventory is disclosed.
• A tibial component of a knee joint prosthesis having a stem which is offset immediately below a tibial tray is disclosed.

DETAILED DESCRIPTION OF VARIOUS EMBODIMENTS
• With initial reference toFIG. 1, a knee joint prosthesis constructed in accordance with the teachings of a preferred embodiment of the present invention is illustrated and generally identified atreference number10. Theknee joint prosthesis10 is generally shown to include atibial component12 and afemoral component14. Thetibial component12 supports abearing16 which engages anarticulation surface18 of thefemoral component14. Insofar as the present invention is concerned, it will be understood that thefemoral component14 and thebearing16 shown inFIG. 1 are conventional in construction.
• Thetibial component12 illustrated inFIG. 1 will be understood to be modular in construction and generally include astem20, atray22, and afirst adapter assembly24. In a manner which will be discussed more fully below, theadapter assembly24 connects thetray22 and thestem20 so as to provide an offset to thestem20 in the transverse plane. Explaining further, when thestem20 is attached to thetray22 through thefirst adapter assembly24, a central axis of thestem20 is offset from acentral axis27 of a downwardly extendingextension30 of thetray22. In the embodiment illustrated, thefirst adapter assembly24 provides a first offset of approximately 5 mm. It will become apparent below that the offset can be in any direction in the transverse plane.
• With brief reference toFIGS. 2 and 3, second andthird adapter assemblies28 and30 according to the teachings of the preferred embodiment of the present invention are illustrated, respectively. The second andthird adapter assemblies28 and30 are shown connecting thetray22 and stem20 ofFIG. 1. As will be discussed more fully below, thesecond adapter assembly28 provides a second offset which in the embodiment illustrated is approximately 2.5 mm. The third adapter assembly is aneutral adapter assembly30 and does not provide any offset. Explaining further, thecentral axis26 of thestem20 is aligned with thecentral axis27 of the downwardly extendingextension29 of thetray22. It will be appreciated by those skilled in the art that the particular degrees of offset provided by thevarious adapter assemblies24,28, and30 of the present invention are strictly a matter of design choice. Alternate offsets will be understood to fall within the scope of the present invention.
• With continued reference toFIG. 1 and additional reference toFIGS. 4 through 10, thefirst adapter assembly24 will be further described. Thefirst adapter assembly24 is illustrated to generally include anadapter body32, a lockinginsert member34 and astem insert member36. Theadapter body32 of thefirst adapter assembly24 is shown to include a first generallycylindrical cavity38 for receiving the downwardly extendingextension29 of thetray22 and a second generallycylindrical cavity40 for receiving and upwardly extendingextension42 of thestem20. The first generallycylindrical cavity38 includes a firstcentral axis42 and the second generallycylindrical cavity40 includes a second generallycylindrical axis44. In the embodiment illustrated, the firstcentral axis42 and the secondcentral axis44 are parallel to one another and spaced apart. Insofar as thefirst adapter assembly24 provides a 5 mm offset, the first and secondcentral axes42 and44 are spaced apart 5 mm.
• The first generallycylindrical cavity38 includes afirst portion46 for directly receiving the downwardly extendingextension29 of thetray22 and a second reduceddiameter portion48 which receives the lockinginsert34. Thefirst portion46 preferably tapers slightly as it extends into theadapter body32 from a top end of theadapter body32. The second generallycylindrical cavity40 similarly includes afirst portion50 and asecond portion52 of reduced diameter. Thefirst portion50 preferably tapers slightly as it extends into theadapter body32 from alower end54 of theadapter body32. Thesecond portion52 of the second generallycylindrical cavity40 is shown to intersect thesecond portion48 of the first generallycylindrical cavity38. In a manner to be described further below, thestem insert36 is partially disposed within thefirst portion50 and extends into thesecond portion52 where it engages the lockinginsert member34.
• With particular reference toFIG. 10, thestem insert member36 is illustrated to include alower portion60 which is externally threaded for engaging an internally threaded aperture of the upwardly extendingextension42 of thestem20. Thestem insert member36 further includes acentral portion62 having a hexagonal or other suitable cross-section which can be engaged by a tool (not shown) for rotating thestem insert member36 into thestem20. Further, thestem insert member36 includes anupper end64 including anenlarged diameter head66 which extends into thesecond portion52 of the second generallycylindrical cavity40.
• With particular reference to the cross-sectional views ofFIGS. 8 and 9, the lockinginsert member34 will be further described. The lockinginsert member34 includes an upper portion having an internally threadedaperture68 and having a square, hexagonal or other suitable cross section that can be engaged by a tool (not shown). The internally threadedaperture68 threadably receives afastener70 which extends through acentral aperture72 provided in thetray22. The lockinginsert member34 additionally includes aradially extending segment74 for engaging thehead66 of thestem insert member36.
• Upon selection by the surgeon of thefirst adapter assembly24, thestem insert member36 is screwed into thestem20. Next, theadapter body32 is placed over the upwardly extendingextension42 of thestem20 such that the upwardly extendingportion42 is received in a press fit within thefirst portion50 of the first generallycylindrical aperture40 and theupper end64 of thestem insert member36 extends into the reduced diametersecond portion52 of the second generallycylindrical cavity40. At this point, the lockinginsert member34 is inserted into the first generallycylindrical cavity38 with theradially extending segment74 opposite the side of the reduceddiameter portion48 which intersects the reduceddiameter portion52. Upon complete insertion, the lockinginsert member34 is rotated approximately between 180° and 270° such that theradially extending portion74 engages theenlarged head66 of thestem insert member36.
• Theadapter body32 is rotated about theaxis27 to provide the offset in the desired direction. Thefirst portion46 of the first generallycylindrical cavity38 is now press fit onto the downwardly extendingextension29 of thetray22. Thestem20 is secured to thetray22 by the threadedfastener70 which extends through theaperture72 and threadably engages the internally threadedaperture68 of the lockinginsert member34. Rotation of the threadedfastener70 in a clockwise direction causes the lockinginsert member34 to be drawn towards thetray22 and a secure connection to be established between thetray22 and thestem20.
• With reference now toFIGS. 2 and 11 through14, thesecond adapter assembly28 of the present invention will now be described. Thesecond adapter assembly28 is illustrated to generally include anadapter body80, a lockinginsert member82 and astem insert member84. Thestem insert member84 is identical to steminsert member36 described above.
• Theadapter body80 of thesecond adapter assembly28 is shown to include a first generallycylindrical cavity86 for receiving the downwardly extendingextension29 of thetray22 and a second generallycylindrical cavity88 for receiving the upwardly extendingextension42 of thestem20. The first generallycylindrical cavity86 includes a firstcentral axis90 and the second generallycylindrical cavity88 includes a second generallycylindrical axis92. In the embodiment illustrated, the firstcentral axis90 and the secondcentral axis92 are parallel to one another and spaced apart. Insofar as thesecond adapter assembly80 provides a 2.5 mm offset, the first and secondcentral axes90 and92 are spaced apart 2.5 mm.
• The first generallycylindrical cavity86 includes afirst portion94 for directly receiving the downwardly extendingextension29 of thetray22 and a second reduceddiameter portion96 which receives the lockinginsert82. As with thefirst adapter assembly24, thefirst portion94 preferably tapers slightly as it extends into theadapter body80 from a top end. The second generallycylindrical cavity88 similarly includes afirst portion98 and asecond portion100 of reduced diameter. Thefirst portion98 preferably tapers slightly as it extends into theadapter body80 from alower end102 of theadapter body80. Thesecond portion100 of the second generallycylindrical cavity88 is shown to intersect thesecond portion96 of the first generallycylindrical cavity86.
• With particular reference to the cross-sectional views ofFIGS. 13 and 14, the lockinginsert member82 will be further described. The lockinginsert member82 includes an upper portion having an internally threadedaperture104 and having a square, hexagonal or other suitable cross section that can be engaged by a tool. The internally threadedaperture104 threadably receives thefastener70 which extends through thecentral aperture72 in thetray22. The lockinginsert member82 additionally includes aradially extending segment106 defining acavity108 for engaging thehead66 of thestem insert member36. Theaperture108 includes a non-cylindrical opening for receiving thehead66 of thestem insert member36 and retaining thehead66 upon rotation in the manner discussed above with respect to thefirst adapter assembly24.
• With reference now toFIGS. 3 and 15 through17, thethird adapter assembly30 of the present invention will now be described. Thethird adapter assembly30 is illustrated to generally include anadapter body110 and alocking insert member112. Theadapter body110 of thethird adapter assembly30 is shown to include a first generallycylindrical cavity114 for receiving the downwardly extendingextension29 of thetray22 an a second generallycylindrical cavity116 for receiving the upwardly extendingextension42 of thestem20. The first generally cylindrical cavity includes a firstcentral axis118 and the second generally cylindrical cavity includes a second generallycylindrical axis120. In the embodiment illustrated, the firstcentral axis118 and the secondcentral axis120 are coincident as thethird adapter assembly30 does not provide any offset.
• The first generallycylindrical cavity114 includes afirst portion122 for directly receiving the downwardly extendingextension29 of thetray22 and a second reduceddiameter portion124 which receives thelocking insert112. Thefirst portion122 preferably tapers slightly as it extends into theadapter body110 from an upper end. The second generallycylindrical cavity116 similarly includes afirst portion126 and a second portion128 of reduced diameter. Thefirst portion126 preferably tapers slightly as it extends into theadapter body110 from alower end130 of theadapter body110. The second portion128 of the second generallycylindrical cavity126 is shown to communicate with thesecond portion124 of the first generallycylindrical cavity114.
• With particular reference toFIG. 17, the lockinginsert member112 is illustrated to include alower portion132 which is externally threaded for engaging the internally threaded aperture of the upwardly extendingextension42 of thestem20. The lockinginsert member112 further includes acentral portion134 and anupper portion136. The upper portion has a square, hexagonal or other suitable cross section which can be engaged by a tool (not shown) for rotating the lockinginsert member112 into thestem20. The internally threadedaperture138 threadably receives thefastener70 which extends through thecentral aperture72 provided in thetray22.
• Turning toFIG. 18, a modular femoral component for a knee joint prosthesis of the present invention is generally identified atreference200. The embodiment ofFIG. 18 illustrates application of the teachings of the first preferred embodiment of the present invention adapted to a modularfemoral component200. The modularfemoral component200 includes an articulatingmember202, astem204 and anadapter assembly206. While not shown in great detail, it will be understood by those skilled in the art that theadapter assembly206 is substantially identical to thefirst adapter assembly24 described above. In this regard, theadapter assembly206 connects the articulatingmember202 and thestem204 and provides an offset between an upwardly extending extension208 of the articulating member and adownwardly extension210 of thestem204. Theadapter assembly206 will be understood to include an adapter body, locking insert member and stem insert member substantially identical to that described above with respect to thefirst adapter assembly24. Alternatively, it will be understood that the adapter assembly of the modularfemoral component200 may be similar to either of the second andthird adapter assemblies28 and30.
• With reference toFIGS. 19, 20 and21A through21C, a tibial component for a knee joint prosthesis constructed in accordance with the teachings of a second preferred embodiment of the present invention is illustrated and generally identified atreference number302. It will be understood that the knee joint prosthesis further includes a femoral component that cooperates with thetibial component302. The particular construction of the femoral component is beyond the scope of the subject invention. One suitable femoral component is, however, shown in connection with the first preferred embodiment.
• Thetibial component302 of the second preferred embodiment of the present invention will be understood to be modular in construction and generally include astem304, atray306, and anadapter assembly308. In a manner which will be discussed more fully below, theadapter assembly308 connects thetray306 and thestem304 so as to provide an offset to thestem304 in the transverse plane. Explaining further, when thestem304 is attached to thetray306 through theadapter assembly308, a central axis of thestem304 is offset from a central axis of a downwardly extendingextension310 of thetray306. In the embodiment illustrated, theadapter assembly308 provides an offset of approximately 5 mm. As with the first preferred embodiment, the offset provided by theadapter assembly308 preferably ranges from 0 mm to approximately 5 mm or more and can be in any direction in the transverse plane.
• Theadapter assembly308 is illustrated to generally include anadapter body312 and a locking member orelement314. Theadapter body312 of theadapter assembly308 is shown to define afirst cavity316 for receiving the downwardly extendingextension310 of thetray306 and asecond cavity318 for receiving and upwardly extendingextension320 of thestem304. In the preferred embodiment, the first andsecond cavities316 and318 are generally cylindrical. Thefirst cavity316 includes a first central axis and thesecond cavity318 includes a second cylindrical axis. Further, in the embodiment illustrated, the first central axis and the second central axis are parallel to one another and spaced apart. Insofar as theadapter assembly308 provides a 5 mm offset, the first and second central axes are spaced apart 5 mm.
• Thefirst cavity316 tapers slightly as it extends into theadapter body312 from atop end326 of theadapter body312. Thesecond cavity318 similarly tapers slightly as it extends into theadapter body312 from alower end322 of theadapter body312. Theadapter body312 is illustrated to further define a laterally extendingchannel324 which intersects both thefirst cavity316 and thesecond cavity318. In a manner to be described further below, the lockingelement314 extends into the laterally extendingchannel324 where it couples thetray306 to thestem304.
• As shown inFIGS. 19 and 20, thestem304 is illustrated to include an upper portion that cooperatively engages with the lockingelement314. In the embodiment illustrated, the upper portion of thestem304 includes a stem insert member. Alternatively, the upper portion of thestem304 may be integrally formed to cooperate with the lockingelement314.
• Thestem insert member330 is illustrated to include alower portion332 which is externally threaded for engaging an internally threadedaperture334 of the upwardly extendingextension320 of thestem304. Thestem insert member330 further includes acentral portion336 having a hexagonal or other suitable cross-section which can be engaged by a tool (not shown) for rotating thestem insert member330 into thestem304. Further, thestem insert member330 includes an upper end including anenlarged diameter head338.
• The lockingelement314 is sized and configured to be inserted through anopening340 in the sidewall of theadapter body312 and into thechannel324 for coupling of thestem304 and thetray306. The lockingelement314 includes an upper surface (seeFIG. 19) having an internally threadedaperture342. The internally threadedaperture342 threadably receives afastener344 which extends through acentral aperture346 provided in thetray306. Thefastener344 is aligned with the central longitudinal axis of the downwardly extendingportion310 of thetray306.
• The lockingelement314 is illustrated to additionally include anopen end350 and a bottom surface having aslot352. Theslot352 intersects theopen end350. Theopen end350 receives thehead338 of thestem insert330 as the lockingelement314 is inserted through theopening340. Theslot352 accommodates the reduced diameter,central portion336 of thestem insert330. Thehead338 of thestem insert330 has a diameter greater than a width of theslot352 for coupling of thestem insert330 with the lockingelement314.
• The lockingelement314 further includes aclosed end356. Theclosed end356 is preferably convexly curved. When thelocking element314 is completely inserted into thechannel324, theclosed end356 is flush with the sidewall of theadapter body312.
• In use, thestem insert member330 is screwed into thestem304. Next, theadapter body312 is placed over the upwardly extendingextension320 of thestem304 such that the upwardly extendingportion320 is received in a press fit within thesecond aperture318 and the upper end of thestem insert member330 extends into the laterally extendingchannel324.
• Thefirst cavity316 is now press fit onto the downwardly extendingextension310 of thetray306 with theadapter body312 oriented to provide the offset in the desired direction. At this point, the lockingelement314 is inserted into the laterally extendingchannel324 through theopening340. Upon complete insertion, the lockingelement314 engages thestem insert member330. Thetray306 is secured to theadapter body312 by the threadedfastener344 which extends through theaperture346 and threadably engages the internally threadedaperture342 of thelocking element314.
• While the invention has been described in the specification and illustrated in the drawings with reference to a preferred embodiment, it will be understood by those skilled in the art that various changes may be made and equivalents may be substituted for elements thereof without departing from the scope of the invention as defined in the claims. In addition, many modifications may be made to adapt a particular situation or material to the teachings of the invention without departing from the essential scope thereof. Therefore, it is intended that the invention not be limited to the particular embodiment illustrated by the drawings and described in the specification as the best mode presently contemplated for carrying out this invention, but that the invention will include any embodiments falling within the description of the appended claims.

Claims (26)

1. A modular tibial component for a knee joint prosthesis, the modular tibial component comprising:

a tray having a support surface and a first extension;

a stem having a main body portion and a second extension;

an adapter body connecting the tray and the stem and establishing a relative offset between the first extension of the tray and the second extension of the stem, the adapter body defining a first cavity to receive the first extension of the tray and a second cavity to receive the second extension of the stem, the adapter body further including a sidewall defining an opening; and

a locking mechanism for coupling the tray to the stem, the locking mechanism including a first locking element passing through the opening and coupling the tray and the stem.

2. The modular tibial component for a knee joint prosthesis ofclaim 1, wherein the first cavity is generally cylindrical and includes a first central axis and the second cavity is generally cylindrical and includes a second central axis, the first and second central axes being parallel to one another and spaced apart.

3. The modular tibial component for a knee joint prosthesis ofclaim 1, wherein the locking element rotatably receives a fastener extending through the tray.

4. The modular tibial component for a knee joint prosthesis ofclaim 1, wherein the locking element includes a slot for receiving a portion of the stem as the locking element is inserted through the opening.

5. The modular tibial component for a knee joint prosthesis ofclaim 4, wherein the portion of the stem received in the slot is integrally formed with the stem.

6. The modular tibial component for a knee joint prosthesis ofclaim 1, wherein the stem includes a reduced diameter portion, the locking element engaging the reduced diameter portion.

7. The modular tibial component for a knee joint prosthesis ofclaim 1, wherein the locking element includes an open end and a bottom surface having a slot intersecting the open end and wherein the stem includes a head received in the open end and having a diameter greater than a width of the slot.

8. The modular tibial component ofclaim 1,

wherein the locking mechanism includes a second locking element;

wherein the second locking element is operable to pass through at least a portion of one of the first cavity, the second cavity, or combinations thereof to engage the first locking element.

9. The modular tibial component ofclaim 8, wherein the first locking element is operable to interact with the second locking element to draw the tibial tray toward the second locking element.

10. The modular tibial component ofclaim 9, wherein the first locking element includes a first thread and the second locking element includes a second thread wherein the first thread and the second thread are operable to interact.

11. A modular component for implanting in a portion of an anatomy, comprising:

a tray having a support surface and a first extension;

a stem having a body portion and a second extension;

an adapter operable to connect the tray and the stem, the adapter defining an internal cavity having a first portion to receive the first extension and a second position to receive the second extension, a passage defined by an outer wall of the adapter; and

a locking member operable to pass through at least a portion of the passage defined by the adapter to operably interconnect the tray and the stem.

12. The modular component ofclaim 11, further comprising:

a second locking member operable to interconnect the tray and the locking member.

13. The modular component ofclaim 12, wherein the locking member threadably receives the second locking member.

14. The modular component ofclaim 8, wherein the first portion of the cavity includes a first central axis and the second portion of the cavity includes a second central axis, the first central axis is at least one of parallel to, spaced apart from, or combinations thereof relative to the second central axis.

15. The modular component ofclaim 11, wherein the outer wall is a side wall.

16. The modular component ofclaim 15, wherein the locking component is operable to pass through the passage substantially orthogonal to an axis of at least one of the first extension, the second extension, or combinations thereof.

17. The modular component ofclaim 11, wherein the joint component is a tibial tray.

18. A modular component for a joint prosthesis comprising:

a joint component;

a stem having an extension;

an adapter defining a first void extending relative to a first axis and a second void extending relative to a second axis; wherein the first void and the second void are defined by a side wall;

a passage defined by the side wall;

a locking member operable to pass through the passage and relative to the first axis, the second axis, or combinations thereof;

wherein the locking element is operable to engage at least one of the tray, the first extension, or combinations thereof to assist in compressing the tray relative to the stem.

19. The modular component ofclaim 17, further comprising:

a second locking element operable to interconnect the tray and the locking element.

20. The modular component ofclaim 17, wherein the joint component is a tibial tray and the stem is an intramedullary stem operable to extend form the tibial tray.

21. The modular component ofclaim 20, further comprising:

a femoral component comprising:

a joint component having a male extension;

a stem for engaging a canal of a bone, the stem having a main body portion and a male extension;

an adapter body for establishing a relative offset between the male extension of the joint component and the male extension of the stem, the adapter body including a first cavity receiving the male extension of the joint component and a second cavity receiving the male extension of the stem, the first cavity intersecting the second cavity; and

a locking arrangement for coupling the tray to the stem, the locking arrangement extending between the first and second cavities, the locking arrangement including a first member removably engaging the tray and a second member removably engaging the stem, the first and second members rotatably coupled to one another within the adapter body.

22. The modular component ofclaim 19, wherein the locking element threadably receives the second locking element extending through the joint component.

23. The modular component ofclaim 19, wherein second locking element is at least one of a screw, a bolt, a quick connect lock, or combinations thereof operable to extend relative to the tray, the system, or the adapter body.

24. The modular component ofclaim 18, wherein the first axis and the second axis are spaced apart.

25. The modular component ofclaim 18, wherein the first axis and the second axis are spaced apart by about 0 mm to about 5 mm.

26. The modular component ofclaim 18, wherein the first axis and the second axis are parallel.
