(54) ТРОАКАР(54) TROAKAR
Изобретение относитс к медицинской технике, а именно хирургическим инструментам , предназначенным дл лапароцентеза . Известен троакар, который содержит гильзу и стилет с руко ткой 1. Однако при использовании известного троакара у больных с ожирением или отеком передней брюшной стенки не всегда возможно определить момент прокола парпетуальной брюшины, в результате чего возникает веро тность травматизации внутренних органов. Целью изобретени вл етс предупреждение травмировани внутренних органов при лапароцелтезе. Поставленна цель достигаетс тем, что троакар, содержащий гильзу и стилет с руко ткой , снабжен подпружиненной втулкой, размещенной между стилетом и гильзой. На фиг. 1 изображен троакар, разрез; на фиг. 2 - то же, в момент прохождени через брюшную стенку; на-фиг. 3 - то же, после прокола брюшной стенки; на фиг. 4 - то же, в момент проведени лечебных манипул ций; на фиг. 5 - вид А на фиг. 1; на фиг. 6 - сечение Б-Б на фиг. 1. Троакар состоит из стилета 1, подпружиненной втулки 2, гильзы 3, пружины 4, упора 5, расположенного на конце втулки 2, руко тки 6, жестко соединенной со стилетом. Причем подпружиненна втулка 2 размещена между стилетом 1 и гильзой 3. Троакар используют следующим образом. Перед пункцией брюшной полости при асците кожу, в месте прокола под местной анестезией рассекают скальпелем на 1 - 1,5 см и троакар ввод т в тело больного. Под действием давлени тканей 7 втулка 2 смещаетс в сторону руко тки 6, сжима пружину 4, и открывает острие стилета 1. После рассечени стенки живота и прокола брюшины втулка 2 под действием пружины 4 возвращаетс в исходное положение и закрывает острие стилета 1. По положению упора 5 суд т о моменте срабатывани пружины. Затем стилет с руко ткой вместе с подпружиненной втулкой и пружиной извлекают, а гильза остаетс в брюшной стенке дл выведени жидкости или поискового лапароцентеза. Описываемый троакар дл лапароцентеза исключает возможность травмировани внутренних органов брюшной полости в момент прокола брюшной стенки.The invention relates to medical technology, namely surgical instruments intended for laparocentesis. The aim of the invention is to prevent trauma to the internal organs during laparoceltosis. The goal is achieved by the fact that a trocar containing a sleeve and a stylet with a handle is provided with a spring-loaded sleeve placed between the stylet and the sleeve. FIG. 1 shows a trocar incision; in fig. 2 - the same at the moment of passage through the abdominal wall; in FIG. 3 - the same, after puncture of the abdominal wall; in fig. 4 - the same, at the time of carrying out medical manipulations; in fig. 5 is a view A of FIG. one; in fig. 6 is a section BB in FIG. 1. A trocar consists of a stylet 1, a spring-loaded sleeve 2, a sleeve 3, a spring 4, an emphasis 5 located at the end of the sleeve 2, a handle 6 rigidly connected to the stylet. Moreover, the spring-loaded sleeve 2 is placed between the stylet 1 and the sleeve 3. The trocar is used as follows. Before puncture of the abdominal cavity in case of ascites, the skin at the puncture site under local anesthesia is dissected with a scalpel in 1-1.5 cm and the trocar is inserted into the patient's body. Under the action of the pressure of the fabrics 7, the sleeve 2 moves towards the handle 6, compressing the spring 4, and opens the tip of the stylet 1. After dissection of the abdominal wall and puncture the peritoneum, the sleeve 2 returns to its original position under the action of spring 4 and closes the tip of the stylet 1. By the position of the stop 5 judges the moment the spring is triggered. Then the stylet with the handle together with the spring-loaded sleeve and the spring is removed, and the sleeve remains in the abdominal wall for fluid withdrawal or exploratory laparocentesis. The described trocar for laparocentesis eliminates the possibility of injury to the internal organs of the abdominal cavity at the time of puncture of the abdominal wall.
