Description

Translated fromJapanese

The present invention relates to a sheath dilator and a catheter assembly using the same.

Nutritional administration methods for patients who cannot take nutrition through the mouth are generally intravenous nutritional administration, transgastric nutritional administration by inserting a tube from the nose into the stomach, etc., and enteral administration from the gastrostomy Three kinds of nutrition administration are performed.
In recent years, enteral nutritional management by percutaneous endoscopic gastrostomy (PEG) has been frequently performed due to the development of enteral nutrients and their administration methods.

As a gastrostomy catheter in PEG, a balloon-type or bumper-type (Malekotto-type) catheter is mainly used.

As a method for percutaneously placing a catheter or the like into a body organ, there is a method using a sheath dilator generally called an introducer method. The sheath dilator includes a sheath that connects the inside of the body organ to the outside of the body, and a dilator that is inserted into the sheath and expands a hole formed between the body surface and the body organ.

A sheath dilator is disclosed in which a sheath is combined with a dilator having a tapered tip (see, for example, Patent Document 1). However, when a catheter is passed through the sheath after diameter expansion (dilation), only the sheath is disclosed. It is necessary to pull out the dilator leaving

When a conventional catheter is placed percutaneously in a body organ, the fistula is first expanded (dilation) with a sheath dilator, and then the dilator is removed from the sheath while the sheath is left in the fistula, and then placed. Since the operation of passing the catheter through the sheath is required, the procedure is complicated and time-consuming.
It is an object of the present invention to provide a sheath dilator and a catheter assembly using the sheath dilator that can easily place a catheter in a short time without having to take out the dilator from the sheath placed after dilation of the fistula.

Next, a sheath dilator and a catheter assembly using the sheath dilator according to a preferred embodiment of the present invention will be described with reference to FIGS.

FIG. 1 shows a catheter assembly (10) according to a preferred embodiment of the present invention. The sheath dilator (11) of the present invention and a catheter (20) set in the sheath dilator (11). The handle (30) is used as a handle when the sheath dilator (11) is divided.
The guide wire (40) is inserted into the lumen of the catheter (20) to serve as a guide for dilation.

The sheath dilator (11) of the present invention has a substantially cylindrical sheath portion (15) and a handle portion (30) used when tearing the sheath portion (15).

The material of the sheath dilator (11) of the present invention is not particularly limited as long as it is a material that can be divided by inflating the balloon (21) at the distal end of the catheter (20) as shown in FIG. For example, a fluororesin such as PTFE or EFP, a polypropylene resin, a polyethylene resin, a polyester resin, and the like can be mentioned, and a fluororesin can be preferably used from the viewpoint of splittability and strength.

On the distal end side of the sheath dilator (11), there is a distal tapered portion (16) that tapers toward the distal end. . By doing so, smooth dilation can be performed. Furthermore, it is preferable that the taper has a constant taper without a step. By doing so, smooth dilation with less resistance can be performed.
The tapered end portion (16) can be formed by a method in which a heated inside of a sheath portion (15) described later is pressed into a cone-shaped mold. Although it can be formed integrally from a tube-like material in this way, it can also be formed as a separate part and bonded using a known technique.

Furthermore, as shown in FIG. 4, at least two weak parts are formed on the circumferential wall surface of the distal end tapered part (16) of the sheath dilator (11) of the present invention and the sheath part (15) in the vicinity of the handle part (30). (12).
By having at least two weak portions on the circumferential wall surface of the tip tapered portion (16), the catheter (20) is pushed through the sheath portion (15) to the tip tapered portion (16) or as shown in FIG. The tip tapered portion (16) can be divided by a balloon (20) attached to the tip of the catheter (20).

。  Furthermore, as shown in FIG. 3, by pulling the handle (22) of the sheath dilator (11), the fragile portion (12) of the sheath portion (15) can be torn and the sheath portion (15) can be divided. .

As the weak part (12), for example, a slit (13) as shown in FIG. 4 can be formed in the outer wall of the sheath part (15). Further, the slit (13) may be formed on the outer wall of the tip tapered portion (16) and the sheath portion (15) as described above, or may be formed on the inner wall.

The fragile portion (12) may further be formed in a thin shape. As an example, as shown in FIG. 5, the lumen (14) can be formed on the wall surfaces of the sheath portion (15) and the tip tapered portion (16). In this way, division can be facilitated.

The fragile portion (12) may be attached to the entire longitudinal direction of the sheath dilator (11) or may be attached to a part thereof. The fragile portion (12) may be formed at the same time as the sheath dilator (11) is molded, or may be formed with a blade or a laser after molding.

The sheath portion (15) and the tapered tip portion (16) are preferably made of a material having orientation in the major axis direction. This makes it easy to divide.

The catheter (20) used in the catheter assembly (10) of the present invention may be any as long as it can be inserted into the lumen of the sheath dilator (11) of the present invention, but the resistance during insertion is low. To balloon catheters are preferred.

Next, an example of a method for using the catheter assembly (10) of the present invention will be described with reference to a gastrostomy procedure.
Gastric wall abdominal wall fixation is performed, a puncture needle is inserted into the center of a quadrangle formed by sutures, and a guide wire is inserted into the stomach through the puncture needle.
After removing the puncture needle, a guide wire is passed through the lumen of the catheter incorporated in the catheter assembly (10) of the present invention, and the entire catheter assembly (10) is inserted into the stomach. After confirming that the sheath part (15) of the catheter assembly (10) has been sufficiently inserted into the stomach with the endoscope inserted into the stomach, the balloon part (21) of the catheter (20) is inflated. Once the tip taper (16) is divided, the catheter (20) is pushed into the stomach.
Next, as shown in FIG. 3, by pulling the handle part (30) left and right, the entire sheath part (15) is pulled out of the body while tearing the sheath part (15).
It is confirmed with an endoscope that the balloon part (21) is inflated, the catheter (20) is pulled out of the body, and the balloon part is fixed in the stomach wall.

As described above, when the catheter assembly of the present invention including the catheter inserted into the lumen of the sheath dilator of the present invention is used, a dilator function is imparted to the sheath portion unlike the conventional sheath dilator. It is not necessary to remove the dilator after inserting the catheter, and the step of inserting the catheter into the sheath becomes unnecessary. By doing so, the object of the present invention of easily placing the catheter in a short time can be achieved.
