Introduction : En radiologie interventionnelle, on se sert assez souvent d'un guide métallique qui permet de garder Un vaisseau ou une voie naturelle (biliaire, urinaire...) accessible à un cathétérisme. Ce guide métallique est mis en place à travers une aiguille de ponction, avec laquelle on a piqué le vaisseau ou la voie naturelle. Problématique : On a assez souvent besoin d'élargir le point d'entrée du guide métallique à la peau. Généralement, on se sert d'un scalpel pour faire une petite incision. Et qui doit se faire au même point d'entrée que le guide. L'innovation : l'idée est de concevoir une lame qui sera guidée sur le guide en place, et permettra ainsi de réaliser une incision précise au même point de ponction ou point d'entrée du guide métallique (voir figure 3). La lame doit avoir une pointe effilée dans le sens du point d'entrée (figure 2). La pointe du système porteur de la lame doit être bien épousée au guide métallique comme est le cas pour le dilatateur d'un désilet monté sur son guide. L'instrument conçu ressemble à un scalpel classique sauf que le bord tranchant est caché dans le système guidé. De même la pointe de la lame est cachée avec un retrait de bord tranchant par rapport au bout de l'instrument. Sur le schéma n° 2 il est mesuré à 2 mm.Introduction: In interventional radiology, a metal guide is often used to keep a vessel or a natural pathway (biliary, urinary, etc.) accessible to catheterization. This metal guide is put in place through a puncture needle, with which we stitched the vessel or the natural way. Problem: We often need to widen the point of entry of the metal guide to the skin. Generally, a scalpel is used to make a small incision. And that must be at the same point of entry as the guide. Innovation: The idea is to design a blade that will be guided on the guide in place, and will thus make a precise incision at the same point of puncture or entry point of the metal guide (see Figure 3). The blade should have a tapered point in the direction of the entry point (Figure 2). The tip of the system carrying the blade must be well matched to the metal guide as is the case for the dilator of a deilet mounted on its guide. The designed instrument looks like a classic scalpel except that the sharp edge is hidden in the guided system. Similarly, the tip of the blade is hidden with a sharp edge withdrawal from the tip of the instrument. In figure 2 it is measured at 2 mm.
