Description

HERNIA REPAIR PLATE
The present invention relates to a hernia repair plate intended, in particular, for abdominal hernias: inguinal, crural. The hernial constrictions constitute a constriction of the collar of the hernia preventing its reduction and leading to intestinal obstruction.

Such hernias can be congenital, result from the weakening of the abdominal wall or from the lesion of this wall following a trauma. They usually manifest as a soft swelling that can be reduced by pressure. The hernial sac is a diverticulum of the peritoneum which engages in the hernial path and contains the prolapsed viscera.

Currently, three techniques are used for hernioplasties or radical cures of a hernia by repairing the sac and consolidating the wall - suturing the muscular wall by laparotomy, - open techniques of the STOPPA, MAHORNAIR type, etc. using prostheses, - laparoscopy necessarily using a prosthesis.

In the case of the use of a prosthesis generally in the form of a plate, it must be fixed to ensure, during the first phase of healing a hold without risk of displacement. The fixing means used currently consist of sutures or
staples. But the realization of sutures or the installation of staples prolong the duration of the operation.

The object of the present invention is to overcome this drawback and to propose a self-supporting repair plate, which is simple and quick to install.

We know that during laparoscopic hernioraphy operations, a gas is introduced into the abdomen which swells it and allows the necessary operating space to be created. After the operation, the gas is evacuated and the abdomen returns to its usual position. The present invention uses this operating mode to maintain the prosthesis which usually takes the form of a generally rectangular and flexible plate.

According to the present invention, the repair plate for an abdominal hernia, flexible and generally rectangular in shape, is characterized in that it comprises a central orifice intended for the passage of the cord, a slot and a flap which is superimposed on a part of the plaque.

Thus, after reduction of the hernia, the plate is slid around the cord by the central opening and the sheet is folded. The deflation of the abdomen or the weight of the viscera then applies the peritoneum against the plate and the latter against the internal face of the abdominal wall.

This pressure prevents the shutter from reopening.

Other characteristics and advantages of the present invention will appear during the description which follows of a particular embodiment, given solely by way of nonlimiting example, with reference to the drawings which represent
- Figure 1, a top view of a plate according to the invention
- Figure 2, a perspective view
In Figure I, we see that the plate I is generally rectangular in shape, the dimensions of which are approximately 15 cm in length and 13 cm in width. It is made of a flexible biocompatible material, in particular polyethylene, polyester, polyamide or polypropylene or absorbable material PGA, PLA .... It has a central circular hole 2 intended to receive the testicular cord and a flap 3 which can be unfolded along a substantially diagonal line 4 to release a slot 5 allowing the opening of the plate for the passage of the cord (not shown).

The mode of use of such a plate is as follows.

Le diamètre du trou 2 est de l'ordre de l cm alors que le diamètre de l'ouverture est de 2 à 3 cm.After which, it folds down the shutter 3, around the line 4 which immobilizes the plate around the cord, the shutter superimposed on the plate 1. This closure simultaneously provides self-fixation and increased tightness around the passage of the cord which constitutes the critical area of this surgery. The diameter of the hole 2 is of the order of 1 cm while the diameter of the opening is 2 to 3 cm.

The connection 4 between the plate I and the flap 3 can be ensured by gluing, welding, sewing, etc. The plate, according to the invention, can be used either in open surgery, or in laparoscopic surgery. In the latter case, the prosthesis is rolled in the hole before being deployed in the cavity.

In another embodiment (not shown), the plate consists of two identical parts, symmetrical after their mounting. They are joined by a center line (sewing, welding or the like) between the upper edge and the central opening. The lower surface of one of the parts, partially cut diagonally, constituting the flap 3 covers the lower surface of the other part so that there is an overlap (without connection) at the bottom of the plate
It goes without saying that many variants can be made, in particular by substitution of equivalent technical means, without departing from the scope of the invention.
