Background
When in laparoscopic surgery, a poking hole is formed on the abdominal wall by using the poking card, so that a channel for the laparoscope and the surgical instrument to enter the abdominal cavity is formed. After the laparoscopic surgery is completed, the stab hole needs to be closed. However, because the stab hole is formed deeper and passes through the skin, subcutaneous fat, the muscular front sheath, the muscle, the muscular rear sheath and the peritoneum, when the stab hole is sutured and closed by using a suture needle outside the body, only the part of the stab hole on the skin, the subcutaneous fat and the front sheath can be sutured and closed, and the part of the stab hole on the muscle, the rear sheath and the peritoneum is difficult to sutured and closed, thus the stab hole hernia is easily formed on the inner side of the abdominal wall, and the herniated small intestine is possibly embedded, even the twisted necrosis endangers the life of the patient.
CN 105640600A discloses a laparoscopic post-operation puncture suturing device for puncture in the year 6 and 8 of 2016, which rotates a curved needle to suture the puncture. Detailed Description
The abdominal wall puncture suturing device according to the present application will be described in detail below with reference to the accompanying drawings.
The abdominal wall stab hole suturing device of the present application comprises a syringe 10 and a needle holder 20.
The cartridge 10 comprises a tubular body, preferably cylindrical. The lower end of the body 10 forms a semicircular shell-shaped end head 13; the tip 13 is hemispherical when viewed from the outside. The hemispherical ends are provided for smooth insertion into the stabbing holes. The tip 13 may have other shapes as long as it is easy to insert into the card-stamping hole.
A partition plate 12 is arranged between the end head 13 and the front end of the body 11.
The upper end of the body 11 is formed with a cover plate 14; a threaded hole 16 is formed through the center of the cover plate 14.
A first long hole 11a and a second long hole 11b are formed in the body in a linear shape along the length direction of the body 11. The first long hole 11a and the second long hole 11b are opposite to each other.
A third long hole 12a and a fourth long hole 12b are formed in the partition plate 12 in a linear shape.
The first long hole 11a and the third long hole 12a communicate with each other; the second long hole 11b and the fourth long hole 12b communicate with each other; the first long hole 11a, the second long hole 11b, the third long hole 12a, and the fourth long hole 12b are on the same plane.
The space between the cover plate 14 and the partition plate 12 is the main space; the space between the partition 12 and the tip 13 is a subspace.
Needle holder 20 includes a first hub 23 and a second hub 24.
The first hub 23 and the second hub 24 are respectively formed in a plate shape. The outer end of the first needle holder 23 is formed with a first cylinder 23b extending vertically upward, and the outer end of the second needle holder 24 is formed with a second cylinder 24b extending vertically upward. The lower ends of the first cylinder 23b and the second cylinder 24b are closed. The first cylinder 23b is used for mounting the first straight needle 31 in a pluggable manner; the second cylinder 24b is used for removably mounting the second straight needle 32.
The tip of the first straight needle 31 is located above the first cylinder 23b, and the tip of the second straight needle 32 is located above the second cylinder 24 b.
The inner end of the first needle seat 23 forms a first guiding inclined plane; the inner end of second hub 24 forms a second guide ramp as shown in fig. 10.
Needle holder 20 further includes a hub push rod 21; the hub push rod 21 is formed in a cylindrical shape, and has a thread formed on the outer circumferential surface.
The lower end of the needle seat push rod 21 is formed with a frustum-shaped push head 22.
The frustoconical pusher head 22 contacts the first and second guide ramps as shown in fig. 9 and 10.
The lower end of the first needle seat 23 forms a first boss 23a; the lower end of second hub 24 forms a second boss 24a. A rubber band 25 is mounted on the first and second bosses 23a and 24a so that the first and second needle hubs 23 and 24 are close to each other.
The needle holder 20 is installed in the cylinder 10, specifically, the first boss 23a passes through the third long hole 12a and then enters the subspace; the second boss 24a passes through the fourth long hole 12b and then enters the subspace. The rubber band 25 is located in the subspace. The first needle mount 23 and the second needle mount 24 are located in the main space; the frustum-shaped pushing head 22 of the needle seat pushing rod 21 is positioned in the main space; the upper end of the hub push rod 21 protrudes from the screw hole 16 to the outside, and the screw thread of the outer peripheral surface of the hub push rod 21 is engaged with the screw hole 16 of the cover plate 14.
The first straight needle 31 forms a first needle hole 31a at a lower portion thereof for coupling to a first end of the suture 40; second straight needle 32 forms a second needle aperture in a lower portion thereof for coupling to a second end of suture 40.
The first pinhole 31a is located above the first cylinder 23 and the second pinhole is located above the second cylinder 24.
First slide guide grooves are formed on both sides of the third long hole 12a, respectively; second slide guide grooves are formed on both sides of the fourth long hole 12b, respectively.
Correspondingly, first sliding rails protruding outwards are formed on two sides of the lower end of the first needle seat 23; the first sliding rail is slidably arranged in the first sliding groove; second sliding rails protruding outwards are formed on two sides of the lower end of the second needle seat 24; the second slide rail is slidably mounted in the second slide groove. The sliding rail and the sliding groove are designed mainly to ensure that the first needle seat and the second needle seat are smoothly pushed out or received horizontally, and no blocking occurs; the structure of the sliding rail and the sliding groove further ensures the vertical state of the first straight needle and the second straight needle, and the first straight needle and the second straight needle cannot deflect in the sliding process.
A stopper handle 15 is formed at the upper end of the body 11; the stopper handles 15 are located outside the main body, are formed on both sides of the main body 11, respectively, and are integrally formed with the main body 11. The stopper lever 15 is inserted into the punch hole to prevent excessive insertion. Another function of the stop handle 15 is to serve as a handle to facilitate insertion and removal of the abdominal wall punch suture device of the present application into and from the punch.
In one embodiment, the body 11, the partition 12, and the tip 13 are integrally formed. The overall structure in this way is more secure.
In another embodiment, the lower end of the body 11 is screw-coupled to the upper side of the partition 12; the head 13 is screwed to the underside of the partition 12. This has the advantage that the main space and the subspace are easily exposed for easy replacement or adjustment of the parts inside.
The tip 13 may be closed; in another embodiment, the lower end of the tip 13 is formed with a through hole (not shown in the drawing) through which the middle portion of the suture 40 is received in the subspace. In this case it is easier to insert suture 40 into the stab hole with tip 13.
In use, in the standby state, the first straight needle 31 and the second straight needle 32 are housed in the main space, as shown in fig. 1. After the end 13 is inserted into the poking hole together with the suture thread 40, the needle holder push rod 21 is rotated, so that the frustum-shaped push head 22 moves downwards, and in the process of moving downwards the frustum-shaped push head 22, the first needle holder 23 and the second needle holder 24 are pushed out to two sides through the first long hole 11a and the second long hole 11b correspondingly by the first guiding inclined plane and the second guiding inclined plane, and the first straight needle 31 and the second straight needle 32 are positioned outside the main body 11, as shown in fig. 3 and 4. The rubber band 25 under the first needle mount 23 and the second needle mount 24 is tensioned at this time. Operating the body 11 so that the whole device is retracted in the stabbing hole, the closed bottoms of the first and second syringes 23b and 24b push the corresponding first and second straight needles 31 and 32 to pierce the abdominal wall until the tips of the first and second straight needles 31 and 32 are exposed from the surface of the abdominal wall; then, the upper ends of the first straight needle 31 and the second straight needle 32 are clamped by the instrument and pulled out, so that both ends of the suture 40 connected with the lower ends of the first straight needle 31 and the second straight needle 32 are pulled out, and the middle part of the suture 40 is positioned in the abdominal cavity; then, the hub push rod 21 is rotated so that the frustum-shaped push head 22 moves upward, and the first hub 23 and the second hub 24 again enter the main body 11 through the corresponding first long hole and second long hole by the contraction force of the rubber band 25. And (5) the main body is pulled out from the poking hole.
Re-rotating the needle holder push rod 21 so that after the first needle holder and the second needle holder are exposed to the outside of the main body, another set of the first straight needle and the second straight needle prepared in advance is re-inserted into the first cylinder and the second cylinder; then, the needle rotating seat push rod 21 is rotated reversely, and the newly loaded first straight needle and second straight needle are accommodated in the main body; then rotated through an angle, such as 90 degrees, and inserted into the body again through the stab hole, the above steps are repeated with the suture thread placed in another orientation. After the suture is set, all the sutures are tied off on the outside of the abdominal wall, thereby forming a closed suture.
