Pharmaceutical compositions are provided which comprise effective amounts of analgesic to treat a subject, including to reduce or eliminate an adverse effect associated with the analgesic.

IIACKGROUND OF THE INVENTION
[00021 Available pain medications may have adverse effects such as nausea, vomiting, and skin rashes. While such medications may be effective in providing pain relief, many subjects are unable to tolerate recommended dosages needed for effective pain relief because of adverse effects. Accordingly, there remains a need for effective analgesics with reduced adverse effects within the art of analgesic compositions.

DETAILED DESCRIPTION OF THE INVENTION
100151 The present invention is generally directed to compositions comprising of multiple active agents that are useful as therapeutics that alleviate, abate or eliminate an adverse effect associated with opioid and/or non-opioid anagesic agents. In various embodiments of the invention a composition comprises an effective amount of two actives, an effective amount of three active agents, an effective amount of four active agents, an effective amount of five active agents or more than five active agents. An active agent is selected from various classes of drugs, including but not limited to opioid analgesics, non-opioid analgesics, decongestant, expectorant, mucus thinning drugs, antitussiNes, antihistamines or a combination thereof.
100161 Various aspects of the invention are directed to compositions comprising an analgesic (e.g., one analgesic or two, three or more analgesics) and an adverse-effect-reducing active agent (e.g., an antihistamine or antiernetic). Such analgesics can include one or more opioid analgesics, or one or more non-opioid analgesics. In various further embodiments the compositions comprise an antihistamine, or an antiemetic.

Of course, under some circumstances an antitussive is also an analgesic.
100341 Thus in some embodiments the composition comprises aceta.minophen, hydrocodone or oxycodone; the antitussive agent is, for example, dikasetron, doroperidone, meclizine, dronabinol, a benzodiazepine, an anticholinergic, hydrocodone or oxycodone; the agent useful for preventing and/or suppressing adverse effect is, for example, an antihistamine such as prornethazine.

As a consequence, subjects experiencing gastric discomfort such as vomiting will lack the beneficial effects of the opioid analgesic and experience the additional discomfort and enhanced pain associated with vomiting. This problem is solved by also administering promethazine, which reduces side-effects.

For example, for the manufacture of hi-layered tablets, the agents can be dispersed uniformly in one or more excipients, for example, using high shear granulation, low shear granulation, fluid bed granulation, or by blending for direct compression. Diluents, also termed "fillers", can be used to increase the bulk of a tablet so that a practical size is provided for compression. Lubricants can also facilitate tablet manufacture;
non-limiting examples thereof include magnesium stearate, calcium stearate, stearie acid, glyceryl behenate, and polyethylene glycol.
Disintegrants can facilitate tablet disintegration after administration, and non-limiting examples thereof include starches, alginic acid, crosslinked polymers such as, e.g., crosslinked polyvinylpyrrolidone, croscarmellose sodium, potassium or sodium starch glycolate, clays, celluloses, starches, gums and the like.

Surfactants can also included and can be anionic, cationic, amphoteric or nonionic. If desired, the tablets can also comprise nontoxic auxiliary substances such as p1-1 buffering agents, preservatives, e.g., antioxidants, wetting or emulsifying agents, solubilizing agents, coating agents, flavoring agent, and the like.

Claims (76)

What is claimed is:
1. A solid oral pharmaceutical composition, comprising:
a layer formulated for immediate-release that comprises an antiemetic; and a layer formulated for controlled-release that comprises an opioid analgesic and a non-opioid analgesic, wherein the opioid analgesic, the non-opioid analgesic, and the antiemetic are present in a ratio by weight of (1 to 2) : (40 to 45) : (1 to 2), respectively, and wherein the solid oral pharmaceutical composition provides an effective amount of the opioid analgesic and the non-opioid analgesic to treat pain in a subject in need thereof, and an effective amount of the antiemetic to reduce or prevent nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in the subject in need thereof.
2. The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, or oxycodone or a pharmaceutically acceptable salt thereof.
3. The solid oral pharmaceutical composition of claim 2, wherein the hydrocodone or the pharmaceutically acceptable salt thereof, or the oxycodone or the pharmaceutically acceptable salt thereof is present in an amount of 1 to 20 mg.
4. The solid oral pharmaceutical composition of claim 2, wherein the opioid analgesic is hydrocodone or the pharmaceutically acceptable salt thereof, and is present in an amount of 1-15 mg.
5. The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, and is present in an amount of 7.5 mg.
6. The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or the pharmaceutically acceptable salt thereof, and is present in an amount of 5 mg.
7. The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone bitartrate and is present in an amount of 7.5 mg.
8. The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is oxycodone or a pharmaceutically acceptable salt thereof, and is present in an amount of 5 mg, 7.5 mg, or 10 mg.
9. The solid oral pharmaceutical composition of claim 8, wherein the opioid analgesic is oxycodone or a pharmaceutically acceptable salt thereof, and is present in an amount of 5 mg.
10. The solid oral pharmaceutical composition of any one of claims 1, 2, 3, 4, or 6, wherein the opioid analgesic is hydrocodone bitartrate.
11. The solid oral pharmaceutical composition of any one of claims 1, 2, 3, 8, or 9, wherein the opioid analgesic is oxycodone hydrochloride.
12. The solid oral pharmaceutical composition of any one of claims 1 or 3-9, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof.
13. The solid oral pharmaceutical composition of any one of claims 1 or 3-9, wherein the antiemetic is promethazine hydrochloride.
14. The solid oral pharmaceutical composition of any one of claims 1 or 2, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, and is present in an amount of 0.5-60 mg.
15. The solid oral pharmaceutical composition of any one of claims 1-5, 7 or 8, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, and is present in an amount of 12.5 mg.
16. The solid oral pharmaceutical composition of claim 1, wherein the antiemetic is promethazine, and is present in an amount of 11 mg.
17. The solid oral pharmaceutical composition of claim 15, wherein the antiemetic is promethazine hydrochloride, and is present in an amount of 12.5 mg.
18. The solid oral pharmaceutical composition of any one of claims 1-9, wherein the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof.
19. The solid oral pharmaceutical composition of any one of claims 1-9, wherein the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and is present in an amount of 200-600 mg.
20. The solid oral pharmaceutical composition of any one of claims 1-5, 7 or 8, wherein the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and is present in an amount of 325 mg.
21. The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, which are present in a ratio by weight of: 1: 43:
1.7, respectively.

The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, which are present in a ratio by weight of 1: 43.33:

The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, which are present in an amount of 6-8 mg, 200-350 mg, and 5-13 mg, respectively.

The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, which are present in an amount of 7.5 mg, 325 mg, and 12.5 mg, respectively.

The solid oral pharmaceutical composition of claim 1, wherein the opioid analgesic oxycodone or a pharmaceutically acceptable salt thereof, the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof, and the antiemetic is promethazine or a pharmaceutically acceptable salt thereof, which are present in an amount of 7.5 mg, 325 mg, and 12.5 mg, respectively.
26. The solid oral pharmaceutical composition of any one of claims 1-9, 21-25, wherein the solid pharmaceutical composition is a tablet.
27. The solid oral pharmaceutical composition of claim 26, wherein the tablet is a bi-layer tablet.
28. Use of the solid oral pharmaceutical composition of any one of claims 1 to 27 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
29. The use of claim 28, wherein the pain is severe pain.
30. A solid oral pharmaceutical composition, comprising:
a layer formulated for immediate-release comprising an antiemetic, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof that is present in an amount of 12.5 mg or 25 mg; and a layer formulated for controlled-release comprising an opioid analgesic that is present in an amount of 1 mg to 20 mg and a non-opioid analgesic that is present in an amount of 200-600 mg, wherein the solid oral pharmaceutical composition provides an effective amount of the opioid analgesic and the non-opioid analgesic to treat pain in a subject in need thereof, and an effective amount of the antiemetic to reduce or prevent nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in the subject in need thereof.
31. The solid oral pharmaceutical composition of claim 30, wherein the promethazine or the pharmaceutically acceptable salt thereof is present in an amount of 12.5 mg.
32. The solid oral pharmaceutical composition of claim 30, wherein promethazine or the pharmaceutically acceptable salt thereof is present in an amount of 25 mg.
33. The solid oral pharmaceutical composition of any one of claims 30-32, wherein the pharmaceutically acceptable salt of the promethazine is promethazine hydrochloride.
34. The solid oral pharmaceutical composition of claim 30, wherein the pharmaceutically acceptable salt of the promethazine is promethazine hydrochloride.
35. The solid oral pharmaceutical composition of claim 30, wherein the opioid analgesic is hydrocodone.
36. The solid oral pharmaceutical composition of claim 35, wherein the opioid analgesic is hydrocodone bitartrate.
37. The solid oral pharmaceutical composition of claim 35, wherein the opioid analgesic is oxycodone hydrochloride.
38. The solid oral pharmaceutical composition of claim 30, wherein the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof.
39. The solid oral pharmaceutical composition of claim 30, wherein the solid oral pharmaceutical composition is a bi-layer tablet.
40. Use of the solid oral pharmaceutical composition of any one of claims 30 to 39 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
41. The use of claim 40, wherein the pain is severe pain.
42. A solid oral pharmaceutical composition, comprising:
a layer formulated for immediate-release comprising a single antiemetic; and a layer formulated for controlled-release comprising a non-opioid analgesic and an opioid analgesic, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, or oxycodone or a pharmaceutically acceptable salt thereof that is present in an amount of 1 mg to 15 mg, and wherein the non-opioid analgesic is present in an amount of mg, wherein the solid oral pharmaceutical composition provides an effective amount of the opioid analgesic and the non-opioid analgesic to treat pain in a subject in need thereof, and an effective amount of the antiemetic to reduce or prevent nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in the subject in need thereof.
43. The solid oral pharmaceutical composition of claim 42, wherein the opioid analgesic is hydrocodone or the pharmaceutically acceptable salt thereof, and is present in an amount of 7.5 mg.
44. The solid oral pharmaceutical composition of claim 42, wherein the opioid analgesic is hydrocodone or the pharmaceutically acceptable salt thereof, and is present in an amount of 5 mg.
45. The solid oral pharmaceutical composition of claim 43, wherein the opioid analgesic is hydrocodone bitartrate and is present in an amount of 7.5 mg.
46. The solid oral pharmaceutical composition of claim 42, wherein the opioid analgesic is oxycodone or the pharmaceutically acceptable salt thereof is present in an amount of 7.5 mg.
47. The solid oral pharmaceutical composition of claim 42, wherein the opioid analgesic is hydrocodone bitartrate.
48. The solid oral pharmaceutical composition of claim 42, wherein the opioid analgesic is oxycodone hydrochloride.
49. The solid oral pharmaceutical composition of claim 42, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof.
50. The solid oral pharmaceutical composition of claim 42, wherein the antiemetic is promethazine hydrochloride.
51. The solid oral pharmaceutical composition of claim 42, wherein the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof.
52. The solid oral pharmaceutical composition of claim 42, wherein the solid oral pharmaceutical composition is a bi-layer tablet.
53. Use of the solid oral pharmaceutical composition of any one of claims 42 to 52 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
54. The use of claim 53, wherein the pain is severe pain.
55. A solid oral pharmaceutical composition, comprising:
a layer formulated for immediate-release comprising an antiemetic that is present in an amount of 12.5 or 25 mg; and a layer formulated for controlled-release comprising an opioid analgesic and a non-opioid analgesic, wherein the opioid analgesic is present in an amount of 1 mg to 20 mg, and wherein the non-opioid analgesic is acetaminophen or a pharmaceutically acceptable salt thereof that is present in 200-600 mg, wherein the solid oral pharmaceutical composition provides an effective amount of the opioid analgesic and the non-opioid analgesic to treat pain in a subject in need thereof, and an effective amount of the antiemetic to reduce or prevent nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in the subject in need thereof.
56. The solid oral pharmaceutical composition of claim 55, wherein the acetaminophen or the pharmaceutically acceptable salt thereof is present in an amount of 325 mg.
57. The solid oral pharmaceutical composition of claim 55, wherein the opioid analgesic is hydroeodone or a pharmaceutically acceptable salt, or oxycodone or a pharmaceutically acceptable salt thereof.
58. The solid oral pharmaceutical composition of claim 57, wherein the opioid analgesic is hydrocodone bitartrate.
59. The solid oral pharmaceutical composition of claim 57, wherein the opioid analgesic is oxycodone hydrochloride.
60. The solid oral pharmaceutical composition of claim 55, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof.
61. The solid oral pharmaceutical composition of claim 60, wherein the antiemetic is promethazine hydrochloride.
62. The solid oral pharmaceutical composition of claim 55, wherein the solid oral pharmaceutical composition is a bi-layer tablet.
63. The solid oral pharmaceutical composition of claim 57, wherein the pharmaceutically acceptable salt of the oxycodone is oxycodone hydrochloride.
64. The solid oral pharmaceutical composition of claim 57, wherein the oxycodone or the pharmaceutically acceptable salt thereof is present in an amount of 5 mg.
65. The solid oral pharmaceutical composition of claim 57, wherein the pharmaceutically acceptable salt of the oxycodone is oxycodone hydrochloride.
66. Use of the solid oral pharmaceutical composition of any one of claims 55 to 65 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
67. The use of claim 66, wherein the pain is severe pain.
68. A solid oral pharmaceutical composition for treating or preventing pain, and reducing or preventing nausea or vomiting associated with an opioid analgesic, wherein the solid oral pharmaceutical composition comprises an immediate-release bi-layered tablet comprising:
a first layer comprising an antiemetic, wherein the antiemetic is promethazine or a pharmaceutically acceptable salt thereof; and a second layer comprising an opioid analgesic, wherein the opioid analgesic is hydrocodone or a pharmaceutically acceptable salt thereof, and a non-opioid analgesic that is acetaminophen or a pharmaceutically acceptable salt thereof, wherein the opioid analgesic, the non-opioid analgesic, and the antiemetic are present in a ratio by weight of (1 to 2) : (40 to 45): (1 to 2), respectively, and wherein the immediate-release bi-layered tablet provides an effective amount of the antiemetic to reduce or prevent nausea associated with the opioid analgesic to reduce or prevent vomiting associated with the opioid analgesic in a subject in need thereof, and an effective amount of the opioid analgesic and the non-opioid analgesic to treat or prevent pain.
69. Use of the solid oral pharmaceutical composition of claim 68 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
70. The use of claim 69, wherein the pain is severe pain.
71. A solid oral pharmaceutical composition for treating or preventing pain, and reducing or preventing nausea or vomiting associated with an opioid analgesic, wherein the solid oral pharmaceutical composition comprises an immediate-release bi-layered tablet comprising:
a first layer, wherein the first layer comprises promethazine; and a second layer, wherein the second layer comprises: 2.5 to 10 mg of hydrocodone, and 325 to 500 mg of acetaminophen, wherein the immediate-release bi-layered tablet provides an effective amount of the promethazine to reduce or prevent nausea associated with hydrocodone or reduce or prevent vomiting associated with hydrocodone in the subject in need thereof, and an effective amount of the hydrocodone and the acetaminophen to treat pain in the subject in need thereof.
72. Use of the solid oral pharmaceutical composition of claim 71 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
73. The use of claim 72, wherein the pain is severe pain.
74. A solid oral pharmaceutical composition for treating or preventing pain, and reducing or preventing nausea or vomiting associated with an opioid analgesic, wherein the solid oral pharmaceutical composition comprises an immediate-release bi-layered tablet comprising:
a first layer, wherein the first layer comprises promethazine; and a second layer, wherein the second layer comprises: 2.5 to 10 mg of hydrocodone, and 325 to 500 mg of acetaminophen, wherein the immediate-release bi-layered tablet provides an effective amount of the promethazine to reduce or prevent nausea associated with hydrocodone or reduce or prevent vomiting associated with hydrocodone, and an effective amount of the hydrocodone and the acetaminophen to treat pain.
75. Use of the solid oral pharmaceutical composition of claim 74 for treating pain, and reducing or preventing nausea associated with the opioid analgesic or vomiting associated with the opioid analgesic in a subject in need thereof.
76. The use of claim 75, wherein the pain is severe pain.
