Fixes#87 Debug utPLSQL tests

• AddDebug utPLSQL test for selected tests in
  • the context menu of the Connections window
  • the context menu of the PL/SQL editor and the Worksheet
  • the context menu of the overview table of the realtime reporter
• Add a debug icon to the toolbar of the realtime reporter to debug all tests of a run
• Add a stop icon to the toolbar of the realtime reporter to stop any test run (with or without debug)
• Initial timeout for consumer changed from 1 minute to 1 hour, this allows to spend more time debugging before the first event is returned. This is interesting when debugging the utPLSQL framework itself
  • to the toolbar of the realtime reporter (all tests of a run)
