 The flag `--gh-host` and the environment variable `GITHUB_HOST` can be used to set
 the GitHub Enterprise Server hostname.
 Prefix the hostname with the `https://` URI scheme, as it otherwise defaults to `http://` which GitHub Enterprise Server does not support.
