    Lewis was the son of Jeremiah and Julianna "Jalia" Thompson Ballard. He married Malinda Jane Spangler in 1855. Together, they had 10 children: Jefferson, Alice, Wade, Lewis, Arthur, Alfred, Jerry, Rose, Homer and Minnie.

    He was a Captain of WV Troops during the Civil War. He was captured and imprisoned in Salisbury, NC. He and another inmate escaped and returned to Monroe County on foot. After the war, he held the office of US Commissioner of Internal Revenue, served as a Delegate in the WV Legislature and Sheriff of Monroe County. He owned a nursery, a grist mill, a flouring mill, and a sawmill.

    He was interred at the Peterstown Cemetery, with the Revs. J. E. Thorne and W. F. Lowrance officiating.
