Watch and Wait has become common in rectal cancer management and is generally considered to be a safe approach. This paper highlights the risk of distant metastases in these patients, with an emphasis on those that exhibit a local regrowth. 

This is important information in terms of selecting patients for watch and wait and fully informing them of the risks.
