Andrew Form (born February 3, 1969)^[2] is an American film producer known for producing the filmsFriday the 13th,Teenage Mutant Ninja Turtles,The Purge andA Quiet Place. He is the co-founder of companyPlatinum Dunes along withMichael Bay andBrad Fuller.^[3]

Form began working in the film industry as aproduction assistant for producerJerry Bruckheimer and Bruckheimer's late producing partnerDon Simpson.

Form founded the production companyPlatinum Dunes with filmmakersMichael Bay andBrad Fuller in 2001. Platinum Dunes is known for producing reboots of popular horror films such asThe Texas Chainsaw Massacre (2003),The Amityville Horror (2005),Friday the 13th (2009) andA Nightmare on Elm Street (2010).^[4]

He is also known for producing original film series such asThe Purge,Ouija (2014),A Quiet Place (2018) andits 2020 sequel.^[5][6] He co-produced the television seriesBlack Sails (2014–2017)^[7] andJack Ryan (2018–present).^[8] In 2015,The Hollywood Reporter called him one of the "30 Most Powerful Film Producers in Hollywood".^[9]

In 2018, Form and Fuller startedFully Formed Entertainment together. The company signed a three-year deal withParamount Pictures beginning in 2019.^[5] Form left the venture in 2020, when he entered talks to joinJohn Krasinski'sSunday Night Productions.^[8][10]

He also produced the filmA Quiet Place: Day One.^[11]

Form met actressJordana Brewster on the set ofThe Texas Chainsaw Massacre: The Beginning, which Form produced. They announced their engagement on November 4, 2006,^[12] and married in theBahamas on May 6, 2007.^[13] Their first son was born in September 2013,^[14] and their second in June 2016.^[15] Brewster filed for divorce in mid-2020.^[16] The divorce was finalized in June 2021. His engagement toAlexandra Daddario was announced on December 2, 2021.^[17] The pair married in June 2022.^[18] In July 2024, Daddario announced she was pregnant with her and Form's first child together, after previously suffering a miscarriage.^[19] The child was born on October 31, 2024.^[20]
