431-REF-000370, FLIGHT ASSURANCE PROCEDURE: PERFORMING A FAILURE MODE AND EFFECTS ANALYSIS, NASA/GSFC (10 AUG 1996)., This procedure establishes guidelines for conducting a Failure Modes and Effects Analysis (FMEA) on GSFC spacecraft and instruments.Typical ground rules for an FMEA are given along with anoverview of the technique, principal, step-by-stepinstructions, sample work sheets, and work sheet dataentries. Specific projects must, of course, add to, deleteand otherwise tailor the procedures to conform with theirneeds, objectives, and contractual requirements. That isparticularly true of safety issues or workaround operationalmethods. Although software analysis is outside the scope ofan FMEA, the effects of failure modes at both software andhardware-software interfaces are included.
