O'Brien was born in Cheltenham, Gloucestershire and went to Tauranga Boys' College. He took method acting classes, and then joined several stage productions as an actor. In 1970, he went into the touring production ofHair for nine months, and spent another nine months in the London production. He also appeared inJesus Christ Superstar andThe Unseen Hand. In 1973, he put his first production ofThe Rocky Horror Show opened at the Theatre Upstairs. Within weeks it had become a box-office hit. It was adapted to film two years later asThe Rocky Horror Picture Show and produced a sequel entitledShock Treatment. Other films he appeared in includeFlash Gordon,The Wolves of Willoughby Chase,Spice World,Ever After,Dungeons & Dragons, andElvira's Haunted Hills.
