- In radiology, the tree-in-bud sign is a finding on a CT scan that indicates some degree of airway obstruction. The tree-in-bud sign is a nonspecific imaging finding that implies impaction within bronchioles, the smallest airway passages in the lung. The differential for this finding includes malignant and inflammatory etiologies, either infectious or sterile. This includes fungal infections, mycobacterial infections such as tuberculosis or mycobacterium avium intracellulare, bronchopneumonia, chronic aspiration pneumonia, cystic fibrosis or cellular impaction from bronchovascular spread of malignancy, as can occur with breast cancer, leukemia or lymphoma. It also includes lung manifestations of autoimmune diseases such as Sjögren syndrome or rheumatoid arthritis. Histopathologic studies have shown that the tree-in-bud pattern is caused by demarcation of the normally invisible branching course of the peripheral airways, which usually results from bronchioles being plugged or blocked with mucus, pus or fluid. In addition, dilated and thickened walls of the peripheral airways and peribronchitis can make the affected bronchioles more easily visible, as is seen in patients with cystic fibrosis. (en)
- In radiologia il segno dell'albero in fiore (in inglese tree-in-bud sign) è un reperto osservabile alla tomografia computerizzata indicativo di un certo grado di ostruzione delle vie aeree. Tale segno indica spesso la presenza di un'infezione che si è diffusa a livello endobronchiale e si associa tipicamente alla tubercolosi e alla broncopolmonite. Inoltre si può riscontrare in quadri di neoformazione come può verificarsi con carcinoma mammario, leucemia o linfoma. Include anche manifestazioni polmonari di malattie autoimmuni come la sindrome di Sjogren o l'artrite reumatoide. Studi di hanno mostrato che il segno dell'albero in fiore è determinato da un'aumentata demarcazione del corso delle vie respiratorie distali, normalmente invisibili alla TC, dovuta ad un accumulo di muco, pus o altro fluido. Altri fattori che concorrono alla maggiore visibilità delle vie aeree distali sono eventuali dilatazioni o ispessimenti parietali ed eventuale peribronchite, reperti per altro osservabili nei casi di fibrosi cistica. Представляет собой мелкие центрилобулярные разветвлённые Y- или V-образные структуры толщиной до 1-2 мм с утолщениями на концах. Морфологическим субстратом этих центрилобулярных узелков является утолщение стенок бронхиол и заполнение их расширенного просвета слизью или гноем (что позволяет визуализировать эти невидимые в норме анатомические структуры). Данная КТ-картина может выявляться как при эндобронхиальном распространении инфекционного процесса, так и при неинфекционных заболеваниях (силикоз, антракоз). (ru)
